摘  要:目的 探究C-末端Ⅱ型胶原(CTX-Ⅱ)和骨桥蛋白(OPN)与绝经后女性退变性膝骨关节炎疼痛的相关性。方法 以2019年2月-2021年7月北京市朝阳区中医医院骨伤科收治的96例绝经后女性退变性膝骨关节炎患者为研究对象根据Kellgren-Lawrence分级将患者分为轻度组、中度组与重度组。收集3组患者血清,采用酶联免疫吸附法试剂盒检测血清CTX-Ⅱ、OPN水平。采用视觉模拟评分法(Visual analogue scale, VAS)与西部安大略和麦克马斯特大学骨关节炎指数评分(Western Ontario and McMaster University Osteoarthritis Index, WOMAC)评估3组患者疼痛程度。采用单因素方差分析法分析3组患者临床指标差异,采用Pearson法分析CTX-Ⅱ、OPN水平与其他临床指标的相关性,多元逐步回归模型分析影响VAS、WOMAC评分的临床因素。结果 3组患者随着患者症状加重,血清CTX-Ⅱ、OPN水平与VAS、WOMAC评分均升高,差异有统计学意义(P<0.05),病程、年龄、绝经年龄、BMI差异无统计学意义(P>0.05)。血清CTX-Ⅱ、OPN水平与VAS评分、WOMAC评分均呈显著正相关(P<0.05),CTX-Ⅱ、OPN水平是影响VAS与WOMAC评分的临床指标。结论 CTX-Ⅱ、OPN水平升高与绝经后女性退变性膝骨关节炎患者疼痛呈显著正相关。Objective To explore the correlation between C-terminal typeⅡcollagen(CTX-Ⅱ)and osteopontin(OPN)and pain in postmenopausal women with degenerative knee osteoarthritis.Methods A total of 96 postmenopausal female patients with degenerative knee osteoarthritis admitted to the hospital from February 2019 to July 2021 were selected as research objects.The above patients were divided into mild group,moderate group and severe group according to Kellgren-Lawrence classification.Serum was collected from 3 group patients.Enzyme linked immunosorbent assay kits were used to detect serum CTX-Ⅱand OPN levels.The visual analogue scale(VAS)and Western Ontario and McMaster University Osteoarthritis Index(WOMAC)were used to evaluate the degree of pain in patients.One-way analysis of variance was used to analyze the differences in clinical indicators among the three groups.Pearson was used to analyze the correlation between CTX-Ⅱ,OPN levels and other clinical indicators.Multiple stepwise regression model was used to analyze the clinical factors affecting VAS and WOMAC scores.Results In three groups of the patients,with the aggravation of symptoms,the levels of CTX-Ⅱand OPN,VAS and WOMAC scores were all increased,and the difference was statistically significant(all P<0.05).There were no significant differences in the course of disease,age,menopause age and BMI(all P>0.05).The levels of CTX-Ⅱand OPN were significantly positively correlated with the scores of VAS and WOMAC(all P<0.05).The levels of CTX-Ⅱand OPN were the clinical indicators affecting the scores of VAS and WOMAC.Conclusion Elevated levels of CTX-Ⅱand OPN are significantly positively correlated with pain in postmenopausal women with degenerative knee osteoarthritis.
