按揉法联合热奄包对混合痔术后患者腹胀及尿潴留的预防作用研究

摘　　要：目的 观察按揉法联合热奄包对混合痔术后患者腹胀及尿潴留的预防作用。方法 选择90例混合痔术后患者,按照随机数字表法分为2组,对照组45例予常规治疗,治疗组45例在对照组基础上予按揉法联合热奄包干预,共干预7天。比较2组首次排尿时间、首次排气时间、平均每日排尿量,治疗1、3、5、7天膀胱残余尿量,统计并比较2组腹胀、尿潴留发生率。结果 治疗组首次排尿时间、首次排气时间均短于对照组(P<0.05),平均每日排尿量多于对照组(P<0.05)。治疗1、3、5、7天,治疗组膀胱残余尿量均少于对照组(P<0.05)。治疗组腹胀总发生率4.44%(2/44),尿潴留总发生率为0,对照组分别为28.89%(13/45)、17.78%(8/45),治疗组腹胀、尿潴留总发生率均低于对照组(P<0.05)。结论 混合痔术后患者予以按揉法联合热奄包干预,可有效缩短排尿、排气时间,减少膀胱残余尿量,降低腹胀、尿潴留发生率。Objective To analyze the preventive effect of massage combined with hot packs on abdominal distension and urinary retention in patients with mixed hemorrhoids surgery.Methods A total of 90 patients with mixed hemorrhoids surgery were selected and randomly enrolled into control groups(n=45)and treatment group(n=45).On the basis of conventional therapy,massage method combined with hot pack intervention was given to the treatment group for seven days.The aiming was to compare the first urination time,first exhaust time,average daily urine output,postvoiding residual bladder volume(PVR)at days 1,3,5,7,abdominal distension between the two groups.The incidence of abdominal distension and urinary retention were recorded.Results The first urination time and the first exhaust time in the observation group were significantly shorter than those in the control group(P<0.05),and the average daily urine output in the observation group was significantly higher than that in the control group(P<0.05).The PVR of the observation group were significantly lower than those of the control group at days 1,3,5,7(P<0.05).The total incidence of abdominal distension(4.44%[2/44]vs 28.89%[13/45])and urinary retention(0 vs 17.78%[8/45])in the treatment group were significantly lower than those in the control group(P<0.05).Conclusion For mixed hemorrhoids,postoperative massage combined with hot packs can effectively shorten the urination and exhaust time,reduce PVR,relieve pain,and decreased the incidence of abdominal distension and urinary retention.
