山药百合杏仁粥对肺肾气虚型支气管哮喘合并营养不良患者营养状况的影响

摘　　要：目的 观察中医补益类药膳山药百合杏仁粥干预对肺肾气虚型支气管哮喘合并营养不良患者营养状态的影响。方法 选择80例肺肾气虚型支气管哮喘合并营养不良患者,采取抽签分组法分为试验组40例及对照组40例,对照组予常规饮食干预,试验组在对照组基础上予山药百合杏仁粥,连续9周。比较2组干预前后上臂围(MAC)、肱三头肌皮褶厚度(TSF)、上臂肌围(MAMC)及白蛋白(ALB)、前白蛋白(PA)、转铁蛋白(TRF)水平、生活质量核心量表(QLQ-C30)评分。根据患者整体营养状况主观评估表(PG-SGA)统计2组营养不良率。结果 干预后,2组MAC、MAMC及TSF均较本组干预前增加(P<0.05),且试验组均高于对照组(P<0.05)。干预后,2组ALB、PA、TRF水平均较本组干预前升高(P<0.05),且试验组ALB、PA、TRF水平均高于对照组(P<0.05)。干预后试验组总营养不良率为5.0%(2/40),对照组为27.5%(11/40),试验组低于对照组(P<0.05)。干预后2组QLQ-C30各维度评分均较干预前提高(P<0.05),且试验组均高于对照组(P<0.05)。结论 对肺肾气虚型支气管哮喘合并营养不良患者实施补益类药膳山药百合杏仁粥干预,可以有效改善患者营养状况,提高生活质量。Objective To explore the effect and improvement of Shanyao Baihe Xingren porridge of traditional Chinese medicine(TCM)nourishing medicated diet on nutritional status of asthma patients with malnutrition of lung-kidney-qi deficiency.Methods The subjects of this study were 80 asthma patients with malnutrition of lung-kidney-qi deficiency.A draw grouping method was performed to assigned to observation group(n=40)and control group(n=40).All patients were given conventional diet intervention,the patients in the observation group were additionally given Shanyao Baihe Xingren porridge,the treatment was conducted for nine weeks,aiming to compare mid-arm circumference(MAC),triceps skinfold thickness(TSF),mid-arm muscle circumference(MAMC),Albumin(ALB),prealbumin(PA),transferrin(TRF),The EORTC Core Quality of Life questionnaire(EORTC QLQ-C30)score between groups before and after treatment.The malnutrition rate of the two groups was calculated using Patient-Generated Subjective Global Assessment Short form(PG-SGA SF).Results After intervention,MAC,MAMC and TSF in the two groups were increased compared with those before intervention(P<0.05),and the increase in the observation group was higher than that in the control group(P<0.05).The levels of ALB,PA and TRF in the two groups were higher than those in the group before intervention(P<0.05),and which in the observation group were more pronounced(P<0.05).The total malnutrition rate in the observation group was lower than that in the control group(5.00%[2/40]vs 27.50%[11/40],P<0.05).The scores of QLQ-C30 in the two groups were higher than those before intervention(P<0.05),and which were higher in the observation group than in the control group(P<0.05).Conclusion For asthma patients with malnutrition of lung-kidney-qi deficiency,Shanyao Baihe Xingren porridge of TCM nourishing medicated diet can effectively improve the nutritional status and improve the quality of life.
