参松养心胶囊对老年心力衰竭合并房颤患者心室重构、NT-proBNP及QTd的影响

摘　　要：目的探讨参松养心胶囊辅助对老年心力衰竭合并房颤的治疗效果及对患者心室重构、N氨基末端脑钠肽前体(NT-proBNP)及QT离散度(QTd)的作用。方法选取2019年1月—2022年8月期间安徽省阜阳市肿瘤医院75例住院治疗的老年心力衰竭合并房颤患者,平均年龄(67.5±4.1)岁,将纳入患者按随机数字表法分为观察组(n=37)和对照组(n=38)。对照组接受盐酸胺碘酮片治疗,观察组接受盐酸胺碘酮片联合参松养心胶囊治疗,2组连续治疗8周。评估2组疗效、治疗后心室重构情况及心功能情况,并统计2组药物不良反应发生情况。结果观察组治疗总有效率较对照组高(P<0.05);2组心室重构差异有统计学意义,其中观察组无重构占比较对照组高(P<0.05);2组NT-proBNP、QTd、左心室舒张末期内径(LVDD)较治疗前低,左心室射血分数(LVEF)水平较治疗前高,且观察组NT-proBNP、QTd、LVDD水平较对照组低,LVEF水平较对照组高(P<0.05);2组不良反应总发生率差异无统计学意义(P>0.05)。结论参松养心胶囊可辅助增强对老年心力衰竭合并房颤患者的疗效,抑制患者心室重构,并提高对心功能及NT-proBNP、QTd水平的调节效果,且不增加药物不良反应发生风险。Objective To explore the therapeutic effects of Shensong Yangxin capsule on elderly patients with heart failure complicated with atrial fibrillation,as well as its effects on ventricular remodeling,N-amino terminal brain natriuretic peptide precursor(NT-proBNP)and QT dispersion(QTd).Methods A total of 75 elderly patients with heart failure complicated with atrial fibrillation hospitalized in Cancer Hospital of Fuyang City,Anhui Province from January 2019 to August 2022,with an average age of(67.5±4.1)years,were selected.They were divided into observation group(n=37)and control group(n=38)according to the random number table method.The control group was treated with amiodarone hydrochloride tablets,and the observation group was treated with amiodarone hydrochloride tablets combined with Shensong Yangxin capsule.The two groups were treated continuously for 8 weeks.The therapeutic effect,ventricular remodeling and cardiac function of the two groups were evaluated,and the incidence of adverse drug reactions in the two groups was counted.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).The difference in ventricular remodeling between the two groups was statistically significant,and the proportion of non-remodeling in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of NT-proBNP and QTd and left ventricular end-diastolic inner diameter(LVDD)of the two groups were lower than those of the same group before treatment,and the level of left ventricular ejection fraction(LVEF)was higher than that of the same group before treatment,while the levels of NT-proBNP and QTd and LVDD of the observation group were lower than those of the control group,and LVEF level was higher than that of the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P<0.05).Conclusion Auxiliary treatment with Shensong Yangxin capsule can improve curative effect on elderly pa
