斑马导丝在留置Foley导尿管失败处理中的临床应用

摘　　要：目的评估斑马导丝在Foley导尿管留置失败中的应用效果。方法100例非手术或非外伤首次留置Foley导尿失败的男性患者,按入院顺序分为试验组和对比组,每组50例。试验组采用斑马导丝进行导尿,对比组采用膀胱造瘘术进行导尿。比较两组患者临床指标(操作成功情况、操作时间、出血量、感染发生情况、住院时间及舒适度评分),分析并发症发生情况。结果两组患者操作成功比例及操作时间比较差异无统计学意义(P>0.05);试验组患者出血量(4.9±1.2)ml、感染发生率2.0%、住院时间(8.9±2.0)d及舒适度评分(5.8±1.2)分均优于对比组的(14.7±2.1)ml、16.0%、(9.8±2.0)d、(7.3±1.0)分,差异有统计学意义(P<0.05)。所有患者均未发生尿道损伤、肠道损伤、大出血等严重并发症。结论留置Foley导尿管失败后应用斑马导丝进行导尿效果良好,其操作快速简单,可提高特殊患者的导尿管留置成功率,优势明显,值得在临床上推广和应用。Objective To evaluate the application effect of Zebra guide wire in management of indwelling Foley catheter failure.Methods A total of 100 male patients with non-surgical or non-traumatic first failed indwelling Foley catheter were divided into experimental group and control group according to the order of admission,with 50 cases in each group.The experimental group received Zebra guide wire for urethral catheterization,and the control group received cystostomy for urethral catheterization.The clinical indicators(operation success rate,operating time,blood loss,infection rate,length of hospital stay,comfort score)were compared between the two groups,and the occurrence of complications was analyzed.Results There was no statistically significant difference in operation success rate and operating time between the two groups(P>0.05).The experimental group had blood loss of(4.9±1.2)ml,infection rate of 2.0%,length of hospital stay of(8.9±2.0)d and comfort score of(5.8±1.2)points,which were all better than those of(14.7±2.1)ml,16.0%,(9.8±2.0)d,(7.3±1.0)points in the control group,and the differences were statistically significant(P<0.05).No serious complications such as urethral injury,intestinal injury or hemorrhoea occurred in all patients.Conclusion After the failure of indwelling Foley catheter,the application of zebra guide wire for urethral catheterization has a good effect,and its operation is fast and simple.It can improve the success rate of indwelling catheter for special patients,and has obvious advantages,which is worthy of clinical promotion and application.
