超声多模态评分评估复发性自然流产患者子宫内膜容受性的临床意义

摘　　要：目的探究超声多模态评分评估复发性自然流产患者子宫内膜容受性的临床价值。方法选取120例复发性自然流产患者作为观察组,另选取同期120例已生育无流产病史者作为对照组。两组受检者均进行二维灰阶超声检查、二维彩色多普勒超声检查、三维超声检查及子宫内膜超声多模态评分评估。比较两组一般资料[年龄、体质量指数(BMI)、孕酮、促黄体生成素]及子宫内膜超声多模态评分,并采用受试者工作曲线(ROC曲线)分析超声多模态评分对复发性自然流产患者子宫内膜容受性的诊断效能。结果两组年龄、BMI、孕酮、促黄体生成素水平比较,差异均无统计学意义(P>0.05)。观察组子宫内膜超声多模态评分(10.57±1.96)分低于对照组的(14.41±1.03)分,差异具有统计学意义(P<0.05)。经ROC曲线分析,超声多模态评分诊断复发性自然流产患者子宫内膜容受性的曲线下面积为0.689,灵敏度为82.98%、特异度为80.00%、准确度为86.68%。结论在复发性自然流产患者子宫内膜容受性评估中,超声多模态评分显著降低,可作为其子宫内膜容受性评估指标。Objective To investigate the clinical value of multimodal ultrasonic score in evaluating endometrial receptivity in patients with recurrent spontaneous abortion.Methods A total of 120 patients with recurrent spontaneous abortion were selected as the observation group and another 120 patients with no history of abortion were selected as the control group.Both groups underwent two-dimensional gray-scale ultrasonography,two-dimensional color Doppler ultrasonography and three-dimensional ultrasonography,and endometrial multimodal ultrasound score assessment.The general data[age,body mass index(BMI),progesterone,luteinizing hormone]and results endometrial multimodal ultrasonic score of the two groups were compared,and receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficiency of multimodal ultrasonic score in the diagnosis of endometrial receptivity in patients with recurrent spontaneous abortion.Results There were no statistically significant differences in age,BMI,progesterone and luteinizing hormone levels between the two groups group(P>0.05).The endometrial multimodal ultrasonic score of(10.57±1.96)points in the observation group was lower than that of(14.41±1.03)points in the control group,and the difference was statistically significant(P<0.05).By ROC curve analysis,the area under the curve for diagnosing endometrial receptivity in patients with recurrent spontaneous abortion by multimodal ultrasonic score was 0.689,with a sensitivity of 82.98%,specificity of 80.00%,and accuracy of 86.68%.Conclusion In the evaluation of endometrial receptivity,the multimodal ultrasonic score in patients with recurrent spontaneous abortion significantly decreased,which could be used as an indicator for evaluating endometrial receptivity.
