健康素养、感知医患共情与患者依从性的关系

摘　　要：探讨健康素养、感知医患共情对患者依从性的影响,以及患者信任在其中发挥的中介作用。运用改良版中国患者健康素养量表、诊疗关系共情量表、维克森医师信任量表和患者依从性量表,对安徽省某综合性三甲医院门诊患者进行现场调查。共收集有效问卷720份,相关性分析显示健康素养、感知医患共情、患者信任与患者依从性均具有相关性(r=0.230、0.419、0.380,P<0.01)。结构方程模型结果显示,患者信任在健康素养与患者依从性之间起完全中介作用;患者信任在感知医患共情与患者依从性之间起部分中介作用。提示应采取措施提高患者健康素养水平,重视医生诊疗共情能力提升,促进医患沟通交流,增进医患信任,进而达到提升患者依从性、维护患者健康的目的。This paper explores the effects of health literacy and perceived doctor⁃patient empathy on patient compliance and the mediating role of patient trust.It utilized the modified Chinese Patient Health Literacy Scale,the Empathy Scale for Diagnosis and Treatment Relationships,Wake Forest Physician’s Trust Scale and the Patient Compliance Scale to investigate outpatients in a comprehensive tertiary hospital in Anhui province with 720 valid questionnaires.Correlation analysis showed that health literacy,perceived doctor⁃patient empathy,and patient trust were all correlated with patient compliance(r=0.230,r=0.419,r=0.380,P<0.01).The results of structural equation modeling showed that patient trust completely mediates between health literacy and patient compliance.Patient trust partially mediates between perceived doctor⁃patient empathy and patient compliance.This paper suggests adopting measures to improve patients’health literacy level,paying attention to the improvement of doctors’empathy ability for diagnosis and treatment,promoting doctor⁃patient communication and doctor⁃patient trust,thereby improving patient compliance and maintaining patient health.
