摘 要:目的 分析不同剂量氨基酸静脉输注对低体质量早产儿营养状态和生长发育的影响。方法 选取我院2020年1月至2021年3月出生的102例低体质量早产儿作为研究对象,随机数字表法按照不同剂量的氨基酸静脉输注分为两组,每组51例。低剂量组采用低剂量氨基酸静脉输注治疗,高剂量组采用高剂量氨基酸静脉输注治疗。观察并记录两组营养状态指标[血红蛋白(Hb)、白蛋白(ALB)、前清蛋白(PA)、总蛋白(TP)和尿素氮(BUN)]和生长发育指标[头围增长速度(HCGR)、身长增加速度(LIR)和体质量增加速度(BMIR)]及并发症发生情况,然后进行统计学分析。结果两组患儿干预后Hb、ALB、PA和TP水平均明显升高(P<0.05),高剂量组干预后Hb、ALB、PA和TP水平显著高于低剂量组,差异有统计意义(P<0.05);高剂量组HCGR、LIR和BMIR显著高于低剂量组,差异有统计意义(P<0.05);高剂量组并发症总发生率为7.84%,低剂量组并发症总发生率为25.49%,差异有统计意义(P<0.05)。结论 高剂量氨基酸静脉营养支持更有利于改善低体质量早产儿的营养状态,促进生长发育,降低并发症发生率,值得临床推广应用。Objective To analyze effects of different doses of amino acid intravenous infusion on nutritional status and growth of premature infants with low body mass.Methods From January 2020 to March 2021,102 premature infants with low body mass who were born in our hospital were selected as objects.According to different doses of amino acid intravenous infusion by random number table method,they were divided into two groups with 51 cases in each group.Low-dose group was treated with low-dose intravenous infusion of amino acids,and high-dose group was treated with high-dose intravenous infusion of amino acids.Nutritional status indicators[hemoglobin(Hb),albumin(ALB),prealbumin(PA),total protein(TP)and urea nitrogen(BUN)]and growth and development indicators(head circumference growth rate(HCGR))were observed and recorded.Length increase rate(LIR),body mass increase rate(BMIR)and complications incidence were statistically analyzed.Results After intervention,Hb,ALB,PA and TP were significantly increased(P<0.05),and Hb,ALB,PA and TP in high-dose group were significantly higher than those in low-dose group with statistical significance(P<0.05).HCGR,LIR and BMIR in high-dose group were significantly higher than those in low-dose group with statistically significant differences(P<0.05).Total complications incidence was 7.84%in high-dose group and 25.49%in low-dose group with statistically significant differences(P<0.05).Conclusion High-dose amino acid intravenous nutritional support is more conducive to improving nutritional status of low-weight premature infants,promoting growth and development,and reducing complications incidence,which is worthy of clinical application.
