摘  要:目的观察补肾活血复方对心肌梗死后心力衰竭大鼠心室重构及心肌和结肠组织水通道蛋白(AQP)4表达的影响,基于“肾藏精主水”理论探析其改善心力衰竭的作用机制。方法将60只SD大鼠随机分为空白组15只和实验组45只,实验组采用结扎左冠状动脉前降支联合力竭式游泳、饥饿建立心肌梗死后心力衰竭模型。将造模成功大鼠随机分为模型组、托伐普坦组和补肾活血组,托伐普坦组灌胃托伐普坦溶液1.35 mg/kg,补肾活血组灌胃补肾活血复方煎液15.75 g/kg,空白组和模型组灌胃等体积蒸馏水,连续4周。心脏彩超检测大鼠左心室结构[收缩末内径(ESD)、舒张末内径(EDD)]与功能[射血分数(EF)、短轴缩短率(FS)],ELISA检测血清N端脑钠肽前体(NT-proBNP)含量,HE染色观察大鼠心肌组织和结肠组织形态,Western blot检测心肌组织和结肠组织AQP4蛋白表达。结果与空白组比较,模型组大鼠左心室ESD、EDD增加(P<0.05),EF、FS减小(P<0.05),血清NT-proBNP含量增加(P<0.05);梗死区心肌细胞坏死,存活心肌细胞代偿性肥大,心肌纤维化;结肠黏膜上皮排列紊乱,杯状细胞减少,大量淋巴细胞浸润;心肌组织AQP4蛋白表达升高(P<0.05),结肠组织AQP4蛋白表达降低(P<0.05)。与模型组比较,托伐普坦组和补肾活血组大鼠左心室ESD、EDD减小(P<0.05),EF、FS增加(P<0.05),血清NT-proBNP含量减少(P<0.05);心肌纤维排列较整齐,部分心肌纤维断裂、坏死;结肠黏膜上皮相对完整、排列较整齐,杯状细胞较多,少量淋巴细胞浸润;心肌组织AQP4蛋白表达降低(P<0.05),结肠组织AQP4蛋白表达升高(P<0.05)。结论补肾活血复方可能通过下调心肌梗死后心力衰竭大鼠心肌组织AQP4蛋白表达、上调结肠组织AQP4蛋白表达,减轻心肌与结肠黏膜损伤,改善心功能,延缓心室重构。本研究基于“肾藏精主水”理论揭示补肾活血复方治疗心肌梗死后心力衰竭的作用机制。Objective To observe the effects of Bushen Huoxue Compound on ventricular remodeling and AQP4 of myocardial and colonic tissue in rats with heart failure after myocardial infarction;To discuss the mechanism of improving heart failure based on“kidney storing essence and regulating water”theory.Methods Totally 60 SD rats were randomly divided into 15 rats in the blank group and 45 rats in the experimental group.The rats in experimental group established the model of heart failure after myocardial infarction by ligating the left anterior descending coronary artery combined with exhaustive swimming and starvation.The modeling rats were randomly divided into model group,tolvaptan group and Bushen Huoxue group.Tolvaptan group was gavaged with tolvaptan solution of 1.35 mg/kg,Bushen Huoxue group was gavaged with Bushen Huoxue Decoction of 15.75 g/kg,and the blank group and model group were gavaged with equal volume of distilled water for four weeks.The structure of left ventricular(ESD and EDD)and function(EF and FS)were measured by echocardiography,the level of serum NT-proBNP was detected by ELISA,the myocardial and colonic morphology was observed by HE staining,the expression of AQP4 in myocardial and colonic tissue were detected by Western blot.Results Compared with the blank group,the left ventricular ESD and EDD in the model group increased(P<0.05),EF and FS decreased(P<0.05),the content of serum NT-proBNP increased(P<0.05);the myocardial cells in infarcted area were necrotic,the viable myocardial cells were compensatory hypertrophy,and the myocardium was fibrotic;the epithelium of colonic mucosa was disordered,goblet cells were reduced,and a large number of lymphocytes were infiltrated;the expression of AQP4 protein in myocardial tissue increased(P<0.05),while the expression of AQP4 in colonic tissue decreased(P<0.05).Compared with the model group,the left ventricular ESD and EDD in tovaptan group and Bushen Huoxue group decreased(P<0.05),EF and FS increased(P<0.05),the content of serum NT-proBNP decreased(P<
