摘　　要：目的:探讨血清微小RNA21(miR-21)与老年心力衰竭并发肺部感染的相关性。方法:将我院收治的130例老年心力衰竭患者根据是否发生肺部感染分为感染组(46例)和非感染组(84例),采用Logistic回归法分析影响感染发生的危险因素。结果:相较于非感染组,感染组SII及miR-21水平更高,PNI水平更低(P<0.05);亚组分析显示,NYHA分级或感染程度越高,患者SII及miR-21水平越高,PNI水平越低(P<0.05);Logistic回归法显示,SII高、miR-21高,PNI低、侵入性操作、滥用抗生素是影响感染发生的危险因素。结论:老年心力衰竭合并肺部感染患者血清miR-21水平显著上升,miR-21联合SII、PNI在诊断患者感染有较高的临床价值。Objective:To investigate the correlation between serum microRNA21(miR-21)and pulmonary infection in elderly patients with heart failure.Methods:130 elderly patients with heart failure admitted to our hospital were divided into infection group(46 cases)and non-infection group(84 cases)according to the occurrence of pulmonary infection.Logistic regression method was used to analyze the risk factors affecting the occurrence of infection.Results:Compared with non-infected group,the levels of SII and miR-21 were higher and PNI were lower in infected group(P<0.05);Subgroup analysis showed that the higher the NYHA grade or infection degree,the higher the level of SII and miR-21 in patients,and the lower the level of PNI(P<0.05);Logistic regression showed that high SII,high miR-21,low PNI,invasive operation and abuse of antibiotics were risk factors affecting infection.Conclusion:The level of serum miR-21 in elderly patients with heart failure and pulmonary infection is significantly increased.The combination of miR-21 with SII and PNI has high clinical value in the diagnosis of infection in patients.
