摘 要:目的探讨血清脂质运载蛋白2(LCN2)、摄食抑制因子-1在预测老年2型糖尿病患者骨质疏松症中的价值。方法本研究为回顾性病例对照研究,选取2018年1月至2022年1月烟台市莱阳中心医院内分泌内科收治的98例老年2型糖尿病患者,男57例,女41例,年龄(72.13±3.15)岁,年龄范围为65~82岁。根据患者的骨密度值(T值)分成非骨质疏松组(T值超过-2.5,n=54)和骨质疏松组(T值不超过-2.5,n=44)。另选取同期在烟台市莱阳中心医院体检的50名健康体检者为健康组。采用双能X线吸收测定法对全部患者的腰椎(L1~L4)椎体正位及左髋部股骨颈近端的T值实施测量,利用酶联免疫吸附法对三组纳入者的血清摄食抑制因子-1、LCN2的浓度水平进行检测。结果健康组、非骨质疏松组、骨质疏松组的LCN2水平[(1.14±0.12)mmol/L、(1.06±0.03)mmol/L、(0.96±0.04)mmol/L]比较,差异有统计学意义(P<0.05)。健康组、非骨质疏松组、骨质疏松组的摄食抑制因子-1水平[(3.57±1.52)μg/L、(3.90±1.53)μg/L、(5.43±1.06)μg/L]比较,差异有统计学意义(P<0.05)。骨质疏松组T值(-3.39±0.04)与非骨质疏松组T值(-0.17±0.05)比较,差异有统计学意义(P<0.05)。血清LCN2与老年2型糖尿病患者骨质疏松症T值呈正相关,摄食抑制因子-1与老年2型糖尿病患者骨质疏松症T值呈负相关。LCN2和摄食抑制因子-1预测老年2型糖尿病患者骨质疏松症的曲线下面积(AUC)分别为0.867(95%CI:0.796~0.939)、0.864(95%CI:0.789~0.939)。结论老年2型糖尿病患者骨质疏松症的血清LCN2水平会降低,摄食抑制因子-1水平会升高,老年2型糖尿病患者骨质疏松症T值与血清LCN2呈正相关,与摄食抑制因子-1呈负相关,LCN2和摄食抑制因子-1对预测老年2型糖尿病患者骨质疏松症具有较高的准确性。Objective To investigate the predictive value of lipocalin 2(LCN2)and feeding inhibitor-1 for osteoporosis in elderly type 2 diabetes.Methods In this retrospective case-control study,a total of 98 elderly patients with type 2 diabetes,including 57 males and 41 females,aged(72.13±3.15)years old,ranging from 65 to 82 years old,were selected from the department of Endocrinology of Yantai Laiyang Central Hospital from January 2018 to January 2022.Patients were divided into non-osteoporosis group and osteoporosis group according to their bone mineral density(T value).In addition,50 healthy people who underwent physical examination in Yantai Laiyang Central Hospital during the same period were selected as the healthy group.Dual-energy X-ray absorptiometry was used to measure the T-value of the lumbar vertebra(L1 to L4)in the upright position and the proximal femoral neck of the left hip of all patients.The concentrations of serum feeding inhibitor-1 and LCN2 in the three groups were detected by enzymed-linked immunosorbent assay.Results The levels of LCN2 in healthy group,non-osteoporosis group and osteoporosis group[(1.14±0.12)mmol/L,(1.06±0.03)mmol/L,(0.96±0.04)mmol/L]were statistically significant(P<0.05).The levels of feeding inhibitor-1 in healthy group,non-osteoporosis group and osteoporosis group[(3.57±1.52)μg/L,(3.90±1.53)μg/L,(5.43±1.06)μg/L]were statistically significant(P<0.05).The T-value in osteoporosis group(-3.39±0.04)and non-osteoporosis group(-0.17±0.05)was statistically significant(P<0.05).Serum LCN2 was positively correlated with T-value of osteoporosis in elderly patients with type 2 diabetes,while feeding inhibitor-1 was negatively correlated with T-value of osteoporosis in elderly patients with type 2 diabetes.The area under curve values of LCN2 and feeding inhibitor-1 for predicting osteoporosis in elderly patients with type 2 diabetes were 0.867(95%CI:0.796 to 0.939)and 0.864(95%CI:0.789 to 0.939).Conclusions The serum LCN2 level of osteoporosis in elderly patients with type 2 diabet
