阳和汤方结合温针灸夹脊穴治疗阳虚寒凝型糖尿病周围神经病临床效果观察

摘　　要：目的观察阳和汤方结合温针灸夹脊穴治疗糖尿病周围神经病变(阳虚寒凝型)临床效果。方法将65例糖尿病周围神经病变(阳虚寒凝型)患者(2017年6月~2021年12月)纳入本次研究,根据治疗方法不同以随机数字表法将65例患者分成对照组(32例)与观察组(33例),对照组患者采取西医治疗(控制血糖,口服甲钴胺及维生素B1),观察组患者在对照组治疗基础上结合阳和汤方及温针灸夹脊穴治疗,观察疗效、中医证候积分(主症、次症、舌脉)变化、多伦多临床神经病变评分(TCSS)变化、血清白细胞介素-6(IL-6)及核转录因子-κB(NF-κB)水平变化、感觉神经传导速度(腓总神经与胫神经)及运动神经传导速度(腓总神经与胫神经)变化、不良反应。结果观察组治疗总有效率(93.94%)比对照组(75.00%)高,差异有统计学意义(P<0.05);治疗前,两组患者TCSS评分、IL-6及NF-κB水平、腓总神经与胫神经感觉神经传导速度及运动神经传导速度等指标比较,差异无统计学意义(P>0.05);治疗后各组患者TCSS评分、IL-6及NF-κB水平、腓总神经与胫神经感觉神经传导速度及运动神经传导速度等指标均改善,观察组患者治疗后TCSS评分、IL-6及NF-κB水平、腓总神经与胫神经感觉神经传导速度及运动神经传导速度等指标优于对照组,差异有统计学意义(P<0.05);两组患者均未发现严重不良反应。结论阳和汤方结合温针灸夹脊穴治疗糖尿病周围神经病变(阳虚寒凝型)临床效果显著,且安全可靠。Objective To observe the clinical effect of Yanghe Decoction combined with warm acupuncture and moxibustion jiaji point on diabetic peripheral neuropathy with Yang deficiency cold coagulation type.Methods 65 patients with diabetes peripheral neuropathy(yang deficiency and cold coagulation type)(June 2017 to December 2021)were included in this study.According to different treatment methods,65 patients were divided into the control group(32 cases)and the observation group(33 cases)by random number table method.The patients in the control group were treated with western medicine(blood sugar control,oral mecobalamin and vitamin B1).The patients in the observation group were treated with Yanghe Decoction and warm needle moxibustion jiaji point on the basis of the treatment in the control group.Observe therapeutic effect,changes in traditional Chinese medicine syndrome scores(main symptom,secondary symptom,tongue pulse),changes in Toronto Clinical Neuropathy Score(TCSS),serum interleukin-6(IL-6),and nuclear transcription factors-κB(NF-κB),changes in sensory nerve conduction velocity(common peroneal and tibial nerves)and motor nerve conduction velocity(common peroneal and tibial nerves),and adverse reactions.Results The total effective rate of the observation group(93.94%)was higher than that of the control group(75.00%)(P<0.05);Before treatment,TCSS score,IL-6 and NF-κB levels,sensory nerve conduction velocity and motor nerve conduction velocity of common peroneal nerve and tibial nerve were compared between two groups(P>0.05).After treatment,TCSS score,IL-6 and NF-κB levels,sensory nerve conduction velocity and motor nerve conduction velocity of common peroneal nerve and tibial nerve were improved in all groups.TCSS score,IL-6 and NF-κB levels,sensory nerve conduction velocity and motor nerve conduction velocity of common peroneal nerve and tibial nerve in observation group were better than those in control group(P<0.05);No serious adverse reactions were found in both groups.Conclusion Yanghe Decoction combined w
