不同负荷剂量苯磺酸瑞马唑仑对妇科宫腔镜手术麻醉镇静效果分析

摘　　要：目的探讨不同负荷剂量苯磺酸瑞马唑仑对妇科宫腔镜手术麻醉镇静效果分析。方法选取2022年2月~2022年12月收治的86例采用妇科宫腔镜手术治疗的患者,按照随机数字法分为A组(n=29)、B组(n=29)和C组(n=28),分别采用0.5 mg/(kg·h)、1.0 mg/(kg·h)和1.5 mg/(kg·h)的苯磺酸瑞马唑仑维持麻醉。评估三组患者麻醉镇静效果、血流动力学、麻醉诱导期对BIS值的影响及不良反应发生情况。结果C组和B组的术中补救镇痛例数明显少于A组(P<0.05),C组和B组的苏醒时间明显长于A组(P<0.05),C组的手术中苯磺酸瑞马唑仑用量明显多于A组和B组(P<0.05),三组患者的起效时间统计学差异无统计学意义(P>0.05);B组患者在T0、T1、T2及T3时的HR均低于A组和C组(P<0.05),但三组患者在T0、T1、T2及T3时的SBP、DBP差异无统计学意义(P>0.05);A组、B组、C组在诱导开始至BIS值<60的时间呈下降趋势,且组间比较差异均具有统计学意义(P<0.05),但麻醉诱导期三组之间的BIS值差异无统计学意义(P>0.05);A组的术中体动发生率明显高于B组和C组(P<0.05),三组患者呼吸抑制、恶心呕吐、低血压差异无统计学意义(P>0.05)。结论采用6 mg/(kg·h)苯磺酸瑞马唑仑麻醉诱导后,1.0 mg/(kg·h)的苯磺酸瑞马唑仑维持麻醉相较于0.5 mg/(kg·h)和1.5 mg/(kg·h)苯磺酸瑞马唑仑维持麻醉应用于宫腔镜手术对患者的循环系统影响相对较小,诱导速度较快,安全性较高,对血液动力学的影响相对较小。Objective To investigate the sedative effect of different loading doses of remazolam benzoate on gynecological hysteroscopic operation.Methods A total of 86 patients who underwent gynecological hysteroscopic surgery from February 2022 to December 2022 were selected and divided into group A(n=29),group B(n=29)and group C(n=28)according to random number method.Remazolam benzoate was administered at 0.5 mg/(kg·h),1.0 mg/(kg·h)and 1.5 mg/(kg·h),respectively.The effect of anesthesia and sedation,hemodynamics,the influence of anesthesia induction period on BIS value and the occurrence of adverse reactions were evaluated in the three groups.Results The number of intraoperative relief analgesia cases in group C and group B was significantly less than that in group A(P<0.05),the recovery time in group C and group B was significantly longer than that in group A(P<0.05),the amount of remazolam benzoate in group C was significantly more than that in group A and group B(P<0.05),and there was no statistically significant difference in the onset time among the three groups(P>0.05).The HR of group B at T0,T1,T2 and T3 was lower than that of group A and group C(P<0.05),but there were no significant differences in SBP and DBP among the three groups at T0,T1,T2 and T3(P>0.05).Group A,group B and group C showed a downward trend from the beginning of induction to BIS<60,and the comparison among groups had statistical significance(P<0.05),but there was no significant difference in BIS between the three groups during anesthesia induction period(P>0.05).The incidence of intraoperative body movement in group A was significantly higher than that in group B and group C(P<0.05),and there were no significant differences in respiratory depression,nausea,vomiting and hypotension among the three groups(P>0.05).Conclusion After anesthesia induction with 6mg/(kg·h)remazolam benzoate,maintenance anesthesia with 1.0 mg/(kg·h)remazolam benzoate has relatively less impact on the circulatory system of patients in hysteroscopic surgery compared wi
