摘 要:目的总结数字减影血管造影(DSA)下肾上腺静脉识别要点及对肾上腺静脉采血操作流程优化的探索,进一步提高肾上腺静脉采血(AVS)效率。方法回顾性收集2020年10月~2023年2月陕西省人民医院确诊原发性醛固酮增多症并行AVS的80例患者资料,对其数字减影图片分析,同时总结操作流程经验。结果对右侧肾上腺静脉鉴别从副肝静脉干扰到肾背膜静脉验证总结识别经验。左侧肾上腺静脉识别以鉴别膈静脉为要点。操作流程从穿刺方案、置管顺序、导管改造到抽血方法进行了一系列优化探索,采血成功率达到96%。结论通过总结肾上腺静脉识别要点以及优化操作流程提高了采样成功率。Objective To summarize the key points of adrenal vein recognition under digital subtraction angiography(DSA)and explore the optimization of adrenal vein blood sampling(AVS).Methods The data of 80 patients with primary aldosteronism and AVS in our hospital from October 2020 to February 2023 were retrospectively collected,and their DSA was analyzed.Results Four lessons were summarized in the identification of right adrenal vein.Distinguishing the inferior phrenic vein was key to the identification of left adrenal vein.Procedure optimization including puncture scheme,catheterization sequence,catheter modification and blood drawing method was explored.The success rate reached 96%.Conclusion The sampling efficiency has been improved by summarizing the key points of AVS and optimizing the procedure.
