早产儿纯母乳喂养持续时间及影响因素分析

摘　　要：目的分析早产儿纯母乳喂养持续时间及其影响因素,为早产儿纯母乳喂养质量持续改进提供依据。方法选取2020年10月至2021年1月在长沙市某三甲妇幼保健院新生儿重症监护早产区住院治疗的早产儿为研究对象,按照严格的纳入与排除标准,通过HIS系统提取住院期间早产儿纯母乳喂养持续时间的相关信息,采用独立样本t检验、方差分析及多元线性回归分析探究早产儿纯母乳喂养持续时间的影响因素。结果早产儿纯母乳喂养持续时间平均(3.61±2.43)个月,母亲就业情况、是否双胎、分娩方式、家人是否支持母乳喂养、开奶时间不同的早产儿纯母乳喂养持续时间差异有统计学意义(P<0.05或P<0.01)。多元线性回归分析显示,母亲无业(β’=0.193)、非双胎(β’=0.270)、家人不支持母乳喂养(β’=-0.242)与早产儿纯母乳喂养持续时间有关(均P<0.01)。结论早产儿纯母乳喂养持续时间亟待提高,母亲就业情况、是否双胎、家人是否支持母乳喂养是早产儿纯母乳喂养持续时间的影响因素。Objective To analyze the duration of exclusive breastfeeding in premature infants and its influencing factors,to provide a basis for the continuous improvement of exclusive breastfeeding quality in premature infants.Meth⁃ods According to strict inclusion and exclusion criteria,premature infants who were hospitalized in the NICU preterm birth area of a tertiary maternal and child health care hospital in Changsha City from October 2020 to January 2021 were selected as the research subjects.The information related to the duration of exclusive breastfeeding of premature infants during hospitalization were extracted from the HIS system,and the influencing factors of the duration of exclusive breastfeeding of premature infants were investigated by independent sample t⁃test,analysis of variance,and multiple linear regression analysis.Results The duration of exclusive breastfeeding in premature infants was(3.61±2.43)months.There was a statistically significant difference in the duration of exclusive breast‑feeding between premature infants with different maternal employment,whether twins or not,different delivery mode,whether family members supported breast‑feeding,and different lactation times(all P<0.05 or P<0.01).Multiple linear regression analysis showed that the mother was unemployed(β'=0.193),non‑twin(β'=0.270),and family members did not support breastfeeding(β'=-0.242)were related to the duration of exclusive breastfeeding in premature infants(all P<0.01).Conclusions The duration of exclusive breastfeeding of premature infants needs to be improved urgently.Maternal employment,twins,and family support for breastfeeding are the influencing factors for the duration of exclusive breastfeeding of premature infants.
