活血化瘀方药对血瘀型慢性阻塞性肺病急性加重期的改善作用

摘　　要：目的探讨活血化瘀方药治疗慢性阻塞性肺病疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)血瘀证的临床疗效。方法收集2021年12月1日—2022年11月31日在上海市浦东新区光明中医医院重症监护室住院的136例符合肺功能Gold分级(Ⅱ、Ⅲ级)的AECOPD(血瘀证)患者。不同性别患者分别采用随机数字表法为对照组和试验组。对照组进行西医常规治疗,试验组加用活血化瘀方药,每日1剂,7 d后比较两组患者临床症状、体征、血气分析、炎症指标及凝血指标。SPSS 20.0进行统计差异性分析。结果与对照组相比,试验组中性粒细胞百分含量(P=0.028)、纤维蛋白原(P=0.012)、降钙素原(P=0.011)明显下降。两组治疗后动脉二氧化碳分压、C反应蛋白均下降(P<0.05或P<0.001)。结论中性粒细胞百分含量、纤维蛋白原、降钙素原、动脉二氧化碳分压和C反应蛋白等参与了AECOPD的病理过程,在西药治疗的基础上,附加活血化瘀方药可能通过降低中性粒细胞百分含量、纤维蛋白原、降钙素原、动脉二氧化碳分压和C反应蛋白等水平治疗AECOPD血瘀证。Objective To explore the efficacy of TCM Huoxue Huayu decoction for blood stasis syndrome in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods One hundred and thirty-six patients with AECOPD admitted from December 2021 to November 2022 entered the clinical trial.The TCM diagnosis of patients was blood stasis syndrome and the pulmonary function gradeⅡtoⅢ.Patients were randomly assigned into two groups,in control group patients received conventional Western medicine,and in study group patients received Huoxue Huayu decoction in addition of conventional treatment.After 7 d of treatment,the clinical symptoms,signs,blood gas analysis,inflammatory index and coagulation index were compared between the two groups.SPSS 20.0 was used for statistical analysis.Results Compared with the control group,the percentage of neutrophils(P=0.028),fibrinogen(P=0.012)and procalcitonin(P=0.011)in the study group were significantly lower.Arterial partial pressure of carbon dioxide and C-reactive protein significantly decreased after treatment in both groups(P<0.05 or P<0.001).Conclusion On the basis of conventional treatment,additional TCM Huoxue Huayu decoction may improve blood stasis syndrome in patients with AECOPD.
