鞍区肿瘤的CT与MRI表现及对比分析

摘  要:目的对计算机断层扫描(computed tomography,CT)与磁共振成像(magnetic resonance imaging,MRI)在鞍区肿瘤诊断中的临床表现和应用价值进行研究。方法选取天津市西青医院放射科于2020年5月至2021年5月接诊的48例鞍区肿瘤患者作为研究对象,对所有患者先后实施CT与MRI检测,并将检测结果与病理检查结果进行对比,同时讨论不同鞍区肿瘤在CT与MRI检查下的临床表现。结果与病理检查结果对比,MRI诊断的准确率为93.75%,明显优于CT诊断的检出准确率72.92%,差异有统计学意义(P<0.05)。MRI诊断对脑膜瘤的诊断准确率为92.31%,高于CT诊断的76.92%,差异无统计学意义(P>0.05)。MRI诊断对颅咽管瘤的诊断准确率为90.91%,高于CT诊断的63.64%,差异无统计学意义(P>0.05)。MRI诊断对垂体瘤的诊断准确率为100%,高于CT诊断的77.78%,差异无统计学意义(P>0.05)。MRI诊断对胶质瘤的诊断准确率为83.33%,高于CT诊断的66.67%,差异无统计学意义(P>0.05)。结论在鞍区肿瘤的临床检测与诊断中,CT与MRI均能取得良好的诊断效果,但MRI检出率明显高于CT的检出率,可为临床应用提供借鉴。Objective To study the clinical manifestations and application value of computed tomography(CT)and magnetic resonance imaging(MRI)in diagnosis of sellar tumors.Methods 48 patients with sellar tumors received by the radiology department of Tianjin Xiqing Hospital from May 2020 to May 2021 were selected as research objects.CT and MRI tests were performed on all patients successively,and the test results were compared with the pathological results.Meanwhile,the clinical manifestations of different sellar tumors under CT and MRI examinations were discussed.Results Compared with the results of pathological examination,the accuracy of MRI diagnosis was 93.75%,which was significantly better than 72.92%of CT diagnosis,the difference was statistically significant(P<0.05).The accuracy of MRI diagnosis for meningioma was 92.31%,higher than 76.92%of CT diagnosis,the difference was not statistically significant(P>0.05).The diagnostic accuracy of MRI for craniopharyngioma was 90.91%,higher than 63.64%of CT diagnosis,the difference was not statistically significant(P>0.05).The diagnostic accuracy of MRI diagnosis for pituitary tumor was 100%,higher than 77.78%of CT diagnosis,the difference was not statistically significant(P>0.05).The accuracy of MRI diagnosis for glioma was 83.33%,higher than 66.67%of CT diagnosis,the difference was not statistically significant(P>0.05).Conclusion In the clinical detection and diagnosis of sellar tumors,both CT and MRI can achieve good diagnostic effects,but the detection rate of MRI is significantly higher than that of CT,which can provide reference for clinical application.
