李晶主任“气虚血瘀夹毒论”治疗慢性萎缩性胃炎经验探析

摘　　要：慢性萎缩性胃炎是由于胃黏膜上皮的反复炎症反应导致胃黏膜病变的一种消化系统慢性疾病,主要症状表现为中上腹隐痛不适、纳差、嗳气、食后腹胀等。世界卫生组织于1978年将其归为胃癌前疾病。山西省中医院李晶主任医师认为,慢性萎缩性胃炎的病机之本是脾气虚,病机之标是胃络瘀阻、兼夹毒邪,在治疗慢性萎缩性胃炎时,一则不忘致病之本——脾胃虚弱,用药轻清平和,时时顾护脾胃;二则重视恢复脾胃升降之职,善用消食导滞之品,助中焦运化;三则谨记瘀血阻络贯穿疾病始终,注重理气活血解毒,同时兼顾养血活血,津血同调。临床治疗以“气虚血瘀夹毒论”为指导,以自拟萎胃方为基础,针对患者不同病情临证灵活加减,配合饮食调整、情志调摄,取得了显著疗效。文章通过案例介绍李晶主任“气虚血瘀夹毒论”治疗慢性萎缩性胃炎的经验,以期为临床提供参考。Chronic Atrophic Gastritis(CAG)is a digestive system chronic disease of the gastric mucosa lesions due to repeated inflammatory reactions of the gastric mucosal epithelium.The main manifestations of patients are middle and upper abdominal pain,poor appetite,belching,abdominal distension,etc..World Health Organization classified it as a gastric precancerous disease in 1978.Dr.LI Jing,a chief physician of Shanxi Provincial Hospital of Traditional Chinese Medicine,believes that the pathogenesis of chronic atrophic gastritis is spleen deficiency,gastric and blood stasis.In the treatment of chronic atrophic gastritis,one does not forget the root of the disease-weak spleen and stomach,light and peaceful medication,and always take care of the spleen and stomach.The second is to attach importance to restoring the function of lifting and lowering the spleen and stomach,and make good use of products that can eliminate food and dredge stagnation to promote the transportation of middre energizer.The third is to keep in mind that blood stasis blocks the entire disease,pay attention to rational Qi(气),activate blood and detoxify,and at the same time take into account blood nourishment and blood revitalization.The clinical treatment,guided by the Qi deficiency blood stasis and toxicant theory and based on the self-made stomach prescription,has achieved remarkable curative effects.This paper introduces Dr.LI Jing’s experience in treating chronic atrophic gastritis based on Qi deficiency blood stasis and toxicant theory,in order to provide a reference for clinical practice.
