摘 要:目的 探讨肺炎支原体(mycoplasma pneumoniae,MP)抗体检测在儿科呼吸道感染疾病诊断中的临床价值,为MP感染早期准确诊断提供依据。方法 选取韩城市妇幼保健院2020年4月至2022年4月就诊的呼吸道感染患儿50例为研究组,选取同期健康儿童50例作为对照组。两组儿童均接受肺炎支原体IgM抗体(MP-IgM)、肺炎支原体IgG抗体(MP-IgG)与肺炎支原体总抗体(MP-Ab)检测,比较两组儿童检测结果,评价MP-IgM、MP-Ab、MP-IgG与联合检测的诊断效能,并统计研究组患儿不同病程阶段MP-IgM、MP-IgG、MP-Ab阳性率。结果 研究组MPIgG阳性率(12.00%)、MP-IgM阳性率(64.00%)与MP-Ab阳性率(80.00%)均显著高于对照组(0.00%、16.00%、6.00%),差异有统计学意义(P<0.05)。ROC曲线分析结果显示,肺炎支原体抗体检测对儿科呼吸道感染的诊断效能为MP-IgM+MP-IgG+MP-Ab>MP-Ab>MP-IgM>MP-IgG。3种抗体阳性率均随病程延长而增高,MP-Ab阳性率峰值出现时间晚于其他两种。结论 肺炎支原体抗体检测在儿科呼吸道感染诊断中有一定的价值,多抗体联合检测较单独检测效果更佳,可作为MP相关疾病早期诊疗依据。Objective To explore the application value of mycoplasma pneumoniae(MP)detection in the diagnosis of paediatric respiratory tract infection.Methods Fifty children with respiratory tract infection and fifty healthy children of the same period from April 2020 to April 2022 were selected,and included in the study group and the reference group,respectively.All the two children were examined with mycoplasma pneumoniae IgM antibody(MP-IgM),mycoplasma pneumoniae IgG antibody(MP-IgG)and total mycoplasma pneumoniae antibody(MPAb),and observed and compared the examination results.The diagnostic efficacy of MP-IgM,MP-Ab、MP-IgG and combined detection was evaluated,and the positive rates of MP-IgM,MP-IgG,and MP-Ab in different stages of the study group were statistically analyzed.Results There was no statistically significant difference in the positive rate of MP-IgG between the two groups of children at the time of treatment(P>0.05).The positive rates of MPIgG(12.00%)、MP-IgM(64.00%)and MP-Ab(80.00%)in the study group were significantly higher than those in the control group(0.00%,16.00%,6.00%),with statistically significant differences(P<0.05).The ROC curve analysis results showed that the diagnostic efficacy of mycoplasma pneumoniae antibody detection for pediatric respiratory tract infections is MP IgM+MP IgG+MP Ab>MP Ab>MP IgM>MP IgG.The positive rates of all three antibodies increased with the prolonged disease duration of the patient,and the peak positive rate of MP-Ab appeared later than other two.Conclusion Antibody detection of mycoplasma pneumoniae is of certain value in the diagnosis of pediatric respiratory tract infection.Compared with antibody detection alone,the effect of multiple antibody combination detection is better,which can be used as the basis for early diagnosis and treatment of MPrelated diseases.
