血清ANGPTL2、AMH、HSP70检测在多囊卵巢综合征患者妊娠情况预测中的应用探究

摘　　要：目的探讨血清血管生成素样蛋白2(ANGPTL2)、抗缪勒管激素(AMH)、热激蛋白70(HSP70)检测在多囊卵巢综合征(PCOS)患者妊娠情况预测中的应用价值。方法选取2021年5月至2022年10月商丘市长征人民医院收治的65例PCOS患者为研究对象,促排卵方案治疗3个月经周期,根据是否妊娠分为妊娠组22例、未妊娠组43例。分析两组(入组时、治疗2个月经周期后)血清ANGPTL2、AMH、HSP70水平,及其与内分泌代谢指标[卵泡刺激素(FSH)、黄体生成素(LH)、泌乳素(PRL)、睾酮(T)、孕酮(P)、雌二醇(E2)]相关性。分析血清各指标对妊娠结局的预测价值。结果治疗2个月经周期后妊娠组血清ANGPTL2、AMH、HSP70水平低于未妊娠组(P<0.05);ANGPTL2与LH、PRL、T、P呈正相关,AMH与PRL、T、P、E2呈正相关,HSP70与FSH、T、P、E2呈正相关(P<0.05);联合预测PCOS患者妊娠结局的曲线下面积(AUC)大于单项预测(P<0.05);ANGPTL2、AMH、HSP70高表达患者未妊娠的危险度分别是低表达患者的1.633倍、1.708倍、1.830倍(P<0.05)。结论血清ANGPTL2、AMH、HSP70与PCOS患者妊娠结局有关,联合检测其水平对妊娠情况具有高度预测价值。【Objective】To investigate the value of serum angiopoietin-like protein 2(ANGPTL2),anti-mullerian hormone(AMH),and heat shock protein 70(HSP70)assays in predicting pregnancy status in patients with polycystic ovary syndrome(PCOS).【Methods】Sixty-five patients with PCOS admitted to our hospital from May 2021 to October 2022 were selected for the study,treated with ovulation promotion protocol for 3 menstrual cycles,and divided into 22 cases in the pregnancy group and 43 cases in the non-pregnancy group according to whether they were pregnant or not.Serum ANGPTL2,AMH,HSP70 levels and their correlation with endocrine metabolic indexes[follicular estrogen(FSH),luteinizing hormone(LH),prolactin(PRL),testosterone(T),progesterone(P),estradiol(E2)]were analyzed in both groups(at the time of enrollment and after 2 menstrual cycles of treatment).The predictive value of each of the serum indicators for pregnancy outcome was analyzed.【Results】Serum ANGPTL2,AMH and HSP70 levels were lower in the pregnant group than in the non-pregnant group after 2 menstrual cycles of treatment(P<0.05);ANGPTL2 was positively correlated with LH,PRL,T and P,AMH was positively correlated with PRL,T,P and E2,and HSP70 was positively correlated with FSH,T,P and E2(P<0.05).The AUC of combined prediction of pregnancy outcome in PCOS patients was greater than that of single prediction(P<0.05);the risk of non-pregnancy was 1.633,1.708,and 1.830 times higher in patients with high expression of ANGPTL2,AMH,and HSP70 than in patients with low expression,respectively(P<0.05).【Conclusion】Serum ANGPTL2,AMH,and HSP70 are associated with pregnancy outcome in patients with PCOS,and their combined levels have a high predictive value for pregnancy.
