消融标测导管远端与近端电极电位间期对室性早搏消融的作用

摘  要:目的探讨消融导管近端电极与远端电极电位间期对室性早搏消融的作用。方法纳入2020年12月至2021年12月在重庆医科大学附属第一医院行射频导管消融的右室流出道室性早搏患者,仅选择起源于右室流出道且第1次放电消融60 s后即无室性早搏复发的患者,最终纳入17例患者。将消融靶点周围随机选择上、下、左、右4个标测记录位点,测量记录位点近端电极与远端电极电位局部间期。结果靶点靠上的位点,远端电位常常较小,未纳入比较;靶点靠左共17个位点,远端电极与近端电极局部间期为(3.76±3.56)ms;靶点靠右共17个位点,远端电极与近端电极局部间期为(3.05±3.41)ms;靶点靠下共17个位点,远端电极与近端电极局部间期为(12.94±4.20)ms。靶点靠左及靠右的位点局部间期差异无统计学意义[(3.76±3.56)ms vs.(3.05±3.41)ms,t=0.59,P=0.56],均短于靶点靠下位点的局部间期[(12.94±4.20)ms,F=38.219,P<0.001]。结论右室流出道导管消融靶点靠下位点与靶点左右位点比较,近端电极与远端电极局部间期更长。Objective To explore the role of the interval of distal and proximal mapping electrodes on the ablation of tract premature ventricular complex.Methods The patients with right ventricular outflow tract-premature ventricualr contraction(RVOT-PVC)who underwent radiofrequency catheter ablation in the First Affiliated Hospital of Chongqing Medical University from December 2020 to December 2021 were selected,of which only patients who originated from the right ventricular outflow tract and had no recurrence of premature ventricualr contraction 60 s after the first discharge ablation were selected.A total of 17 patients were finally included.The interval between the proximal and the distal mapping electrodes of the recording site was measured at four randomly selected recording sites around the ablation target site:upper,lower,left and right.Results The distal mapping electrode was often smaller in the upper sites of the target and was not included in the comparison.There were 17 sites to the left,right and lower sites of the target respectively,and the intervals of distal and proximal mapping electrodes was(3.76±3.56)ms,(3.05±3.41)ms and(12.94±4.20)ms,respectively.There was no significant difference between the intervals in the left and right sites[(3.76±3.56)ms vs.(3.05±3.41)ms,T=0.59,P=0.56].They were both significantly shorter than that of lower sites[(12.94±4.20)ms,F=38.219,P<0.001].Conclusion The interval between the proximal and distal mapping electrodes at the right ventricular outflow tract catheter ablation target site is longer compared to the interval between the proximal and distal mapping electrodes at the left and right sites of the target site.
