中老年慢性心力衰竭并胸腔积液通过留置导管引流的疗效评估

摘　　要：目的探讨中老年慢性心力衰竭并胸腔积液患者留置导管引流的疗效。方法选取2019年6月—2020年6月丰台中西医结合医院收治的63例中老年慢性心力衰竭并胸腔积液住院患者为研究对象。按照随机数表法分为两组,对照组30例,观察组33例。对照组患者予限盐、限水、利尿等常规性治疗;观察组在此基础上,给予胸腔留置管持续引流,14 d后比两组治疗效果。结果治疗后,观察组患者在减轻体质量、减慢心率、增加6 min步行试验距离、降低血BNP值以及减少胸腔积液最深水深方面均优于对照组,差异有统计学意义(P<0.05);观察组住院时间为(11.70±1.78)d,明显短于对照组的(13.83±2.63)d,差异有统计学意义(t=6.236,P<0.05)。结论在常规性治疗的基础上给予胸腔留置管引流处理治疗中老年慢性心力衰竭并胸腔积液患者,可以在短时间内安全地改善患者临床症状,缩短住院时间。Objective To investigate the efficacy of drainage by indwelling catheter in middle-aged and elderly patients with chronic heart failure and pleural effusion.Methods Sixty-three middle-aged and elderly inpatients with chronic heart failure and pleural effusion admitted to Fengtai Hospital of Chinese and Western Medicine from June 2019 to June 2020 were selected for the study.The patients were divided into two groups according to the random number table method,30 cases in the control group and 33 cases in the observation group.The patients in the control group were given conventional treatments such as salt restriction,water restriction and diuresis.The observation group was given continuous drainage with a chest tube on this basis.14 d later,the treatment effects of the two group and the control group were compared.Results After treatment,patients in the observation group were better than those in the control group in terms of reducing weight,slowing heart rate,increasing the distance of 6 min walking test,reducing blood BNP value and decreasing the deepest depth of pleural fluid,and the differences was statistically significant(P<0.05).The length of hospital stay in the observation group was(11.70±1.78)d,significantly shorter than that in the control group(13.83±2.63)d,the difference was statistically significant(t=6.236,P<0.05).Conclusion On the basis of conventional treatment,thoracic indentation tube drainage treatment for middle-aged and elderly patients with chronic heart failure and pleural effusion can safely improve clinical symptoms and shorten hospital stay in a short time.
