手术医师信息库在落实围手术期核心制度中的应用

摘　　要：实现围手术期信息化、数字化管理,是落实医疗质量安全核心制度和规范医疗行为的重要保障。通过完善手术分级授权评估体系,建立健全包含手术医师基本信息、手术级别、手术量、医疗负性指标等内容的信息化手术医师库,并整合到医院信息化管理系统中,规范医务人员医疗行为,减少围手术期医疗安全不良事件发生。统计数据显示,手术医师库上线后,全院手术患者并发症发生率下降0.27个百分点,非计划再次手术发生率下降0.02个百分点,围手术期医疗文书书写及时率上升8.19个百分点,Ⅰ类切口围手术期抗菌药物预防使用率降至30%以下。结果表明,基于信息系统建立围手术期质量安全管理体系,是落实医疗质量安全核心制度的重要手段。Implementing perioperative informatization and digital management is an important guarantee for implementing the core system of medical quality and safety and regulating medical behavior.By improving the surgical hierarchical authorization evaluation system,establishing and improving an information-based surgical physician database that included basic information of surgeons,surgical level,surgical volume,and medical negative indicators,and integrating it into the hospital information management system,it aimed to standardize medical behavior of medical personnel and reduce the occurrence of adverse medical safety events during the perioperative period.Statistical data showed that after the launch of the surgical physician database,the incidence of complications in patients undergoing surgery in the entire hospital decreased 0.27 percent point,the incidence of unplanned reoperation decreased 0.02 percent point,the timely completion rate of perioperative documents increased 8.19 percent point,and the prophylactic use rate of antibiotics during the perioperative period of Class I incisions decreased to below 30%.The results indicated that establishing a perioperative quality and safety management system based on information systems is an important measure to further implement the core system of medical quality and safety.
