维持性血液透析患者血清中单核细胞/高密度脂蛋白胆固醇比值、中性粒细胞/淋巴细胞比值与CRP相关性分析

摘　　要：目的探讨维持性血液透析患者血清中单核细胞/高密度脂蛋白胆固醇比值(monocyte-to-high-density lipoprotein cholesterol ratio,MHR)、中性粒细胞/淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)与C反应蛋白(C-reactive protein,CRP)的相关性。方法选取2021年6月至2022年4月在广西医科大学第四附属医院行血液透析的终末期肾病患者83例,收集患者基础数据、CRP、中性粒细胞计数、单核细胞计数、淋巴细胞计数、高密度脂蛋白胆固醇等,从而计算出MHR、NLR并分析其与炎症因子相关性。结果维持性血液透析患者MHR、NLR与CRP具有相关性(P<0.01),MHR与患者中性粒细胞计数、淋巴细胞计数呈正相关(P<0.05),以NLR为因变量,回归模型纳入CRP,以MHR为因变量,回归模型纳入中性粒细胞计数、淋巴细胞计数、年龄、体重。结论血液透析患者MHR、NLR与CRP有相关性,MHR与中性粒细胞计数、淋巴细胞计数有相关性,NLR、MHR可进一步研究作为患者炎症状态指标。Objective To investigate the correlation between serum monocyte-to-high-density lipoprotein cholesterol ratio(MHR),neutrophil-to-lymphocyte ratio(NLR),and C-reactive protein(CRP)in patients undergoing maintenance hemodialysis.Methods This study included a total of 83 end-stage renal disease patients who underwent hemodialysis at the Fourth Affiliated Hospital of Guangxi Medical University from June 2021 to April 2022.Patient demographics,CRP levels,neutrophil counts,monocyte counts,lymphocyte counts,and high-density lipoprotein cholesterol levels were collected to calculate MHR and NLR and analyze their correlation with inflammatory factors.Results There was a significant correlation between MHR,NLR,and CRP in patients undergoing maintenance hemodialysis(P<0.01).MHR was positively correlated with neutrophil counts and lymphocyte counts in these patients(P<0.05).Regression models were employed with NLR as the dependent variable,and CRP as an included factor.Similarly,a regression model with MHR as the dependent variable included neutrophil counts,lymphocyte counts,age,and body weight.Conclusion MHR and NLR are correlated with CRP in patients undergoing maintenance hemodialysis.Moreover,MHR is associated with neutrophil counts and lymphocyte counts.Further research is warranted to explore the potential of NLR and MHR as indicators of inflammatory status in hemodialysis patients.
