摘　　要：目的:基于Ablett分析法对54例成人破伤风患者的临床特征及住院费用进行分析,为破伤风的诊治和预防提供依据。方法:选取2011年5月—2021年5月湖南省人民医院暨湖南师范大学附属第一医院急诊科收治的成人破伤风患者54例为研究对象。对54例成人破伤风患者的流行病学特征、临床特征及住院费用等数据进行分析。结果:54例成人破伤风患者中,平均年龄(55.1±11.9)岁,>40岁49例(90.7%);农民22例(40.7%),进城务工人员7例(13.0%);49例有明确外伤史;平均潜伏期9.0 (4.2,16.8)d;平均住院时间18.0 (13.0,24.8)d;呼吸机使用时间为0(0,11.8)d;住院费用47 670(18 000,76 750)元。Ablett分级为Ⅳ级的破伤风患者住院时间、呼吸机使用时间、住院费用多于Ⅰ、Ⅱ、Ⅲ级,差异有统计学意义(P<0.05)。结论:成人破伤风患者具有明显的年龄和职业特征,Ablett分级可以评估病情严重程度和住院费用。Objective:To analyze the clinical characteristics and hospitalization costs of 54 adult tetanus patients based on Ablett classification,and provide reference for the diagnosis,treatment and prevention of tetanus.Methods:From May 2011 to May 2021,54 adult tetanus patients admitted to the Department of Emergency Medicine in Hunan Provincial People's Hospital and the First Affiliated Hospital of Hunan Normal University were selected as the study subjects.The epidemiological characteristics,clinical characteristics and hospitalization costs of 54 adult tetanus patients were analyzed.Results:Among the 54 adult tetanus patients,the mean age was(55.1±11.9)years,and 49 cases(90.7%)were>40 years old;There were 22 farmers(40.7%)and 7 migrant workers in urban areas(13.0%);49 cases had a clear history of trauma;the mean incubation period was 9.0(4.2,16.8)d;the mean number of hospitalization days was 18.0(13.0,24.8)d;the ventilator use time was 0.0(0.0,11.8)d;Hospitalization costs were 47670(18000,76750)yuan.The number of hospitalization days,ventilator use time,and hospitalization cost of tetanus patients with Ablett grade IV were more than those with gradeⅠ,Ⅱ,Ⅲ,and the difference was statistically significant(P<0.05).Conclusion:Adult tetanus patients have distinct age and occupational characteristics,and Ablett classification can assess disease severity and hospitalization costs.
