摘　　要：目的:观察某三级甲等专科儿童医院门急诊抗菌药物的使用情况,分析不合理处方,并提出改进措施。方法:回顾性分析2020年1月—2021年12月某三级甲等专科儿童医院门急诊开具的1186张抗菌药物处方,分析抗菌药物使用情况,不合理处方占比、构成情况。结果:该专科儿童医院门急诊使用频率排名前3位的抗菌药物为头孢克洛干混悬剂、头孢克肟颗粒、注射用头孢曲松钠;合理处方1175张,不合理处方11张,不合理处方占0.93%;不合理处方中,联合用药无适应证5张,占45.45%;超常处方3张,占27.27%;重复用药2张,占18.18%;遴选药品不适宜1张,占9.10%。结论:该专科儿童医院门急诊存在抗菌药物不合理使用情况,需加强监管,针对不合理用药情况应及时进行干预。Objective:To observe the use of antibacterial drugs in outpatient and emergency department of a tertiary Grade-A specialized children's hospital,analyze the irrational prescriptions and propose the improvement measures.Methods:From January 2020 to December 2021,1186 prescriptions of antibacterial drugs issued by the outpatient and emergency department of a tertiary Grade-A specialized children's hospital were retrospectively analyzed.The use of antibacterial drugs,the proportion and composition of irrational prescriptions were analyzed.Results:According to the use frequency in outpatient and emergency department of this specialized children's hospital,the top three antibacterial drugs were cefaclor for suspension,cefixime granules and ceftriaxone sodium for injection.There were 1175 rational prescriptions and 11 irrational prescriptions,and irrational prescriptions accounting for 0.93%.Among the irrational prescriptions,there were 5 prescriptions without indications of combined drug use,accounting for 45.45%;3 off-label use prescriptions,accounting for 27.27%;2 prescriptions with repeated drug use,accounting for 18.18%;and 1 prescription with inappropriate drug selection,accounting for 9.10%.Conclusion:The irrational use of antibacterial drugs exists in outpatient and emergency department of this specialized children's hospital.It is necessary to strengthen the supervision,and carry out timely intervention for irrational drug use.
