摘 要:目的研究组合渐进式运动对云南高原骨质疏松症干预效果。方法选择2020年7月-2022年7月云南省体育运动创伤专科医院收治的在云南高原居住5年及以上骨质疏松症患者76例,对照组、观察组各38例均予口服基础抗骨松药物,观察组予组合渐进式运动干预,共24周。于干预前、第12周、第24周及结束后4周观察患者BMD T值、Ca^(+)、25-OH-D、CT、ALP、肌力、平衡能力、TUG、SF-36。结果对照组25-OH-D较干预前显著升高(P<0.05),余指标差异无统计学意义(P>0.05);观察组干预后对比干预前及对照组患者BMD T值、肌力、平衡能力、SF-36均有显著提升(P<0.05),ALP、TUG显著下降(P<0.05),血Ca^(+)、CT无明显差异(P>0.05),血25-OH-D较干预前降低,第12周达水平54.81±6.15 nmol/L,第24周、结束后4周较第12周无明显差异。结论组合渐进式运动可有效改善云南高原骨质疏松症患者骨密度、骨代谢、身体功能及生活质量,其量效关系需进一步研究。Objective To observe the intervention effect of combined progressive exercise on osteoporosis in Yunnan Plateau.Methods From July 2020 to July 2022,76 patients with osteoporosis who had lived in Yunnan Plateau for more than 5 years were admitted to Yunnan Provincial Sports Trauma Hospital.38 patients in control group and observation group for each were given oral anti-osteoporosis drug,and observation group were given combined progressive exercise intervention also,lasted for 24 weeks.Bone mineral density(BMD T),blood calcium(Ca^(+)),blood 25 hydroxyvitamin D(25-OH-D),calcitonin(CT),alkaline phosphatase(ALP),muscle strength,balance,fall risk(TUG),and quality of life(SF-36)were observed before intervention,at week 12,week 24,and 4 weeks after intervention.Results 25-OH-D in control group was significantly higher than that before intervention(P<0.05),there was no statistical significance in other indexes(P>0.05);BMD T value,muscle strength,balance and SF-36 were significantly improved in the observation group after intervention compared with those before intervention and those in control group(P<0.05),ALP、TUG significantly decreased(P<0.05),there was no significant difference in blood Ca^(+),CT(P>0.05),blood 25-OH-D was lower than before intervention,the level at week 12 reached 54.81±6.15 nmol/L,and there was no significant difference in the 24th week and 4 weeks after the end compared with the 12th week.Conclusions Combined progressive exercise can effectively improve bone mineral density,bone metabolism,physical function and quality of life for osteoporosis patients in Yunnan Plateau,but its dose-effect relationship needs further study.
