摘　　要：目的探讨关节镜下复位内固定治疗成人新鲜后交叉韧带止点撕脱骨折的方法及疗效。方法回顾分析2019年7月-2022年1月本院收治诊断为新鲜后交叉韧带止点撕脱骨折患者共60例,所有患者采用关节镜下行后交叉韧带止点撕脱骨折复位不可吸收缝线并栓桩固定,对比所有患者术前及术后6个月时的膝关节Lysholm功能评分,记录患者术后3、6个月的症状、体征,通过其术后1、3、6个月的患膝关节X线片观察骨折愈合情况。结果术后6个月复查骨折均愈合良好,术后3、6个月复查患肢后抽屉试验均呈阴性,Lysholm评分由术前(55.23±1.74)分提高至(91.72±0.83)分,膝关节活动度由术前(46.5±5.4)提升至(125±10.7),差异有统计学意义(P<0.05)。结论关节镜下使用不可吸收缝线联合栓桩复位固定治疗胫骨髁后棘骨折疗效显著,是治疗一种有效可靠的治疗方法。Objective To discuss the methods and effects of restoration internal fixation under arthroscope in treatment of adults with avulsion fracture of fresh posterior cruciate ligament.Methods Retrospectively analyzed 60 patients with avulsion fracture of fresh posterior cruciate ligament who recieved the treatment in our hospital from July 2019 to Jan.2022.All the patients underwent non-absorbable suture and peg fixation under arthroscop for posterior cruciate ligament avulsion fracture restoration.The Lysholm function scores of all the patients before surgery and 6 months after surgery were compared,the symptoms and signs of the patients 3 and 6 months after surgery were recorded,and the fracture healing was observed by X-ray of knee joints 1,3 and 6 months after surgery.Results All the fractures healed well 6 months after surgery,and the posteriordrawer test of the affected limb were negative 3 and 6 months after surgery.The score increased from(55.23±1.74)to(91.72±0.83)before surgery,and the range of motion of the knee increased from(46.5±5.4)to(125±10.7)before surgery,and the difference was statistically significant(P<0.05).Conclusions The use of non-absorbable suture combined with peg fixation under arthroscope in treatment of fracture of posterior condylar spine of tibia,and is an effective and reliable treatment method.
