摘　　要：目的构建早期肠造口患者家庭照顾者核心照顾技能指标体系,为制定早期肠造口患者家庭照顾者照顾技能培训方案提供参考依据。方法以Farran的照顾者技能模型为理论指导,通过文献回顾、半结构式访谈、经研究小组讨论制定出早期肠造口患者家庭照顾者核心照顾技能指标体系初稿,并应用德尔菲法对16名专家进行2轮函询,采用层次分析法确定各指标权重。结果2轮专家咨询积极系数分别为100%、94%,权威系数分别为0.885、0.900,专家意见变异系数0~0.164,肯德尔和谐系数分别为0.163、0.312(均P<0.01),最终形成早期肠造口患者家庭照顾者核心照顾技能指标体系,包括一级指标5项,二级指标15项,三级指标48项。结论本研究构建的早期肠造口患者家庭照顾者核心照顾技能指标体系理论依据可靠,研究方法严谨,实用性较高,后期可依据本指标体系制定肠造口患者家庭照顾者核心照顾技能培训方案,提升照顾者照顾能力,改善肠造口患者健康结局。Objective To construct an index system of core care skills for family caregivers of patients with enterostomy in the early stage,and to provide a reference for the development of a caregiving skills training program for family caregivers of patients with early enterostomies.Methods Based on the caregiver skill model proposed by Farran as the theoretical guidance,the first draft of the core caregiving skills index system for family caregivers of patients with early enterostomies was developed through literature review,semi-structured interviews,and discussion by the research team.Sixteen experts were invited for 2 rounds of letter consultation with Delphi method and AHP was used to determine the weight of all the indexes.Results The positive coefficient of the two rounds of expert consultation was 100%and 94%,and the authority coefficient 0.885 and 0.900 respectively.The variation coefficient was 0~0.164,and the Kendall harmony coefficient 0.163 and 0.312 respectively(all P<0.01).The index system of core care skills for family caregivers of patients with enterostomy in the early stage was formed,including 5 first-level,15 second-level,and 48 third-level indicators.Conclusion The index system of core care skills for family caregivers of patients with enterostomy in the early stage constructed in this study is reliable and practical,and the training program of core care skills for family caregivers can be formulated according to this index system in the near future,so as to enhance the caring ability of caregivers and improve the health outcomes of patients.
