摘　　要：目的探讨基于FOCUS-PDCA模型构建的“移动ICU”服务模式在危重患者院际转运质量管理中的应用效果。方法选取德阳市人民医院重症医学科实施院际转运的48名护士为研究对象,2020年7—12月(实施质量改进前)使用传统危重患者院际转运方法,2021年4—9月(实施质量改进后)使用基于FOCUS-PDCA模型构建的危重患者院际转运方案,对实施质量改进前、后护士院际转运综合能力以及113例危重症患者的转运相关不良事件发生率、转运时长进行比较。结果实施质量管理后护士院际转运能力得分明显优于实施前,差异有统计学意义(P<0.05);实施质量管理后的转运前准备时长、抵院后入病区时长均明显优于改进前,差异有统计学意义(P<0.05);质量管理后用物不全、呼吸支持、输注设备、监护设备相关的不良事件发生率均显著降低,差异有统计学意义(P<0.05)。结论基于FOCUS-PDCA模型构建的“移动ICU”护理服务模式能有效的提高护士的院际转运能力,减少不良事件的发生,并提高转运效率。Objective To explore the application effect of"mobile ICU"service model based on FOCUS-PDCA model in quality management of critical patients'inter-hospital transport.Methods 48 nurses from the Department of Critical Care Medicine of Deyang People's Hospital were selected as the research subjects.From July to December 2020(before implementing quality improvement),traditional methods of inter hospital transportation for critically ill patients were used.From April to September 2021(after implementing quality improvement),a FOCUS-PDCA model based inter hospital transportation plan for critically ill patients was constructed.Compared the comprehensive ability of post hospital nurses in inter hospital transportation,as well as the incidence and duration of transportation related adverse events in 113 critically ill patients.Results The score of nurses'inter hospital transfer ability after implementing quality management was significantly better than before implementation,the difference was statistically significant(P<0.05).After implementing quality management,the preparation time before transportation and the admission time after arriving at the hospital were significantly better than before,the difference was statistically significant(P<0.05).After quality management,the rate of adverse events related to incomplete use of materials,respiratory support,infusion equipment,and monitoring equipmentwas significantly reduced,the difference was statistically significant(P<0.05).Conclusion The"mobile ICU"nursing service model based on the FOCUS-PDCA model can effectively improve the nurses'inter-hospital transport ability,reduce the occurrence of adverse events,and improve the transport efficiency,which is worthy of clinical application.
