微创接骨术与续骨活血汤对胫骨平台前内侧塌陷骨折并膝关节后外侧复合体损伤患者结局的影响

摘　　要：目的探讨对胫骨平台前内侧塌陷骨折并膝关节后外侧复合体损伤患者采用微创接骨术联合续骨活血汤治疗对膝关节胫骨平台畸形、下肢外翻角以及并发症的影响。方法随机选择2021年1—12月莱阳市中医医院收治胫骨平台前内侧塌陷骨折合并膝关节后外侧复合体损伤者80例,以随机数表法分为对照组(n=40,微创接骨术治疗)与观察组(n=40,联合续骨活血汤治疗),比较两组患者关节功能、胫骨平台内反角、后倾角、下肢外翻角畸形纠正效果以及并发症发生率。结果观察组膝关节功能优良率高于对照组(92.50%vs75.00%),差异有统计学意义(χ^(2)=4.501,P<0.05);观察组术后内反角小于对照组,后倾角、下肢外翻角大于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论对胫骨平台前内侧塌陷骨折并后外侧复合体损伤患者在微创接骨术治疗同时配合续骨活血汤干预能改善膝关节功能,可预防胫骨平台及内翻畸形,能降低并发症风险。Objective To investigate the effects of minimally invasive bone grafting combined with Xuguhuoxue decoction on tibial plateau deformity,lower extremity varus angle and complications in patients with anterior medial tibial plateau collapse fracture and posterior lateral knee joint complex injury.Methods A total of 80 cases of anterior medial tibial plateau collapse fracture complicated with posterolateral complex injury of knee joint treated in Laiyang Hospital of Traditional Chinese Medicine from January to December 2021 were randomly selected and divided into control group(n=40,minimally invasive bone grafting treatment)and observation group(n=40,combined with Huoxue decoction treatment)by random number table method.The joint function,the internal reverse Angle of tibial plateau,the posterior Angle,the correction effect of lower extremity varus Angle deformity and the incidence of complications were compared between the two groups.Results The excellent and good rate of knee function in the observation group was higher than that in the control group(92.50%vs 75.00%),and the difference was statistically significant(χ^(2)=4.501,P<0.05).The internal inversion Angle of the observation group was lower than that of the control group,and the posterior inclination Angle and lower extremity varus Angle were higher than that of the control group,the difference was statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion For patients with anterior medial collapse fracture of tibial plateau and posterior lateral complex injury,minimally invasive bone grafting combined with Xuguhuoxue decoction intervention can improve knee joint function,prevent tibial plateau and varus deformity,and reduce the risk of complications.
