双侧子宫动脉结扎术联合Cook球囊治疗产后出血的临床研究

摘　　要：目的探讨双侧子宫动脉结扎术联合Cook球囊治疗产后出血的临床效果。方法方便选取2019年4月—2021年4月间厦门大学附属第一医院收治的76例产后出血产妇作为研究对象,依据不同治疗方式分为联合治疗组和对照组,各38例。两组均进行临床常规治疗,对照组给予双侧子宫动脉结扎术治疗,联合治疗组在对照组基础上联合Cook球囊治疗,比较两组治疗后临床效果,记录并比较两组手术治疗中及产后24 h患者出血量及产后并发症发生情况。结果联合治疗组止血成功率(94.74%)比对照组(76.32%)高,子宫切除率(5.26%)比对照组(23.68%)低,差异有统计学意义(χ^(2)=5.208、5.208,P<0.05);联合治疗组中手术时间比对照组长,止血时间比对照组短,术中出血量、术后24 h出血量及输血量比对照组少,差异有统计学意义(P<0.05);治疗后,联合治疗组并发症总发生率(5.26%)比对照组(21.05%)低,差异有统计学意义(χ^(2)=4.145,P<0.05)。结论双侧子宫动脉结扎术联合Cook球囊治疗产后出血疗效显著,有利于成功控制出血量,同时可在一定程度上降低子宫切除发生率,保留产妇正常生育功能。Objective To investigate the clinical effect of bilateral uterine artery ligation combined with Cook balloon in the treatment of postpartum hemorrhage.Methods Seventy-six cases of postpartum hemorrhage treated in the First Affiliated Hospital of Xiamen University from April 2019 to April 2021 were conveniently selected as research subjects,and were divided into the combined treatment group and the control group according to different treatment methods,with thirty-eight cases in each.Both groups received clinical routine treatment,the control group received bilateral uterine artery ligation treatment,and the combined treatment group combined Cook balloon treatment on the basis of the control group.The clinical effect after treatment was compared between the two groups,and the amount of blood loss and the occurrence of postpartum complications were recorded and compared between the two groups.Results The success rate of hemostasis in the combined treatment group(94.74%)was higher than that in the control group(76.32%),and the hysterectomy rate(5.26%)was lower than that in the control group(23.68%),the difference was statistically significant(χ^(2)=5.208,5.208,P<0.05).The operation time and hemostatic time of the combined treatment group was longer than that of the control group,and the amount of intraoperative blood loss,24 h postoperative blood loss and blood transfusion were less than those of the control group,and the difference was statistically significant(P<0.05).After treatment,the total complication rate of the combined treatment group(5.26%)was lower than that of the control group(21.05%),and the difference was statistically significant(χ^(2)=4.145,P<0.05).Conclusion Bilateral uterine artery ligation combined with Cook balloon is effective in the treatment of postpartum hemorrhage,which is beneficial to successfully control the amount of bleeding,and can reduce the incidence of hysterectomy to a certain extent and preserve the normal reproductive function of the women.
