超声引导下肩胛上神经+腋神经阻滞联合盂肱关节臭氧+类固醇注射治疗肩关节痛的疗效评价

摘　　要：目的分析及评价超声引导下肩胛上神经+腋神经阻滞联合盂肱关节臭氧+类固醇注射治疗肩关节痛的疗效。方法回顾性分析2021年6—12月于江苏大学附属医院疼痛科门诊就诊的肩关节痛患者60例。根据治疗方式分为对照组(n=29)和观察组(n=31)。对照组采用超声引导下盂肱关节臭氧+类固醇注射,观察组在对照组基础上联合超声引导下肩胛上神经+腋神经阻滞。评价两组治疗前后视觉模拟评分(VAS)、Constant肩关节功能评分(CSS)、治疗有效率、复发率。结果治疗后,两组VAS评分均比治疗前降低,且观察组治疗2周后、4周后VAS评分均低于对照组,差异有统计学意义(P<0.05);治疗后,两组CSS评分均比治疗前升高,且观察组治疗2周后、4周后CSS评分均高于对照组[(64.00±7.12)分、(73.03±5.97)分vs(51.69±7.20)分、(57.77±6.49)分],差异有统计学意义(t=-6.654,-9.496,P<0.05);治疗后,观察组治疗有效率优于对照组,观察组治疗后4周复发率低于对照组,差异有统计学意义(P<0.05)。结论超声引导下肩胛上神经+腋神经阻滞联合盂肱关节臭氧+类固醇注射可以更有效地缓解肩关节痛,改善肩关节功能。Objective To analyze and evaluate the efficacy of ultrasound-guided suprascapular nerve+axillary nerve block combined with glenohumeral joint ozone+steroid injection in the treatment of shoulder joint pain.Methods A retrospective analysis was performed on sixty patients with shoulder joint pain who were treated in the Department of Pain,Affiliated Hospital of Jiangsu University from June 2021 to December 2021.They were divided into control group(n=29)and observation group(n=31).The control group received ultrasound-guided glenohumeral joint ozone+steroid injection,and the observation group was combined with ultrasound-guided suprascapular nerve+axillary nerve block on the basis of the control group.Visual Analog Scale(VAS),Constant Shoulder Score(CSS),effective rate of treatment and recurrence rate were evaluated between the two groups.Results After treatment,the scores of VAS were lower than before treatment of the two groups,and the scores of VAS of observation group was lower than control group after 2 weeks and 4 weeks,the difference was statistically significant(P<0.05).After treatment,the scores of CSS were higher than that before treatment,and the observation group was higher than the control group after 2 weeks and 4 weeks of treatment[(64.00±7.12)points,(73.03±5.97)points vs(51.69±7.20)points,(57.77±6.49)points],the difference was statistically significant(t=-6.654,-9.496,P<0.05).After treatment,the effective rate of treatment in the observation group was better than that of the control group,and the recurrence rate of the observation group was lower than that of the control group 4 weeks after treatment,the difference was statistically significant(P<0.05).Conclusion Ultrasound-guided suprascapular nerve+axillary nerve block combined with glenohumeral joint ozone+steroid injection can more effectively relieve shoulder pain and improve shoulder joint function.
