二尖瓣成形术与二尖瓣置换术在风湿性二尖瓣瓣膜病变治疗中的疗效分析

摘　　要：目的分析风湿性二尖瓣瓣膜病变应用二尖瓣成形术及二尖瓣置换术的效果。方法回顾性分析2018年1月—2022年1月吉林省人民医院收治的58例风湿性二尖瓣瓣膜病变患者的临床资料,按照治疗方法分为对照组(n=29,二尖瓣置换术治疗)和观察组(n=29,二尖瓣形成术治疗),对比两组手术指标、心功能指标、二尖瓣反流情况及术后并发症发生率。结果观察组手术时间、体外循环时间、术中出血量、住院时间优于对照组,差异有统计学意义(P<0.05)。治疗后,观察组患者的LVEF、LVEDD高于对照组,LVESD、LAD低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为13.79%,低于对照组的48.28%,差异有统计学意义(χ^(2)=8.056,P=0.005)。结论二尖瓣成形术在风湿性二尖瓣瓣膜病变治疗中的疗效更理想,值得在临床治疗中推广应用。Objective To analyze the effect of mitral valvuloplasty and mitral valve replacement in rheumatic mitral valve disease.Methods A retrospective analysis was conducted on the clinical data of 58 patients with rheumatic mitral valve disease admitted to Jilin Provincial People's Hospital from January 2018 to January 2022.They were divided into a control group(n=29,treated with mitral valve replacement surgery)and a observation group(n=29,treated with mitral valve formation surgery)according to the treatment method.The surgical indicators,cardiac function indicators,mitral valve regurgitation,and postoperative complications of the two groups were compared.Results The observation group had better surgical time,extracorporeal circulation time,intraoperative bleeding volume,and hospital stay than the control group,and the difference was statistically significant(P<0.05).After treatment,the LVEF and LVEDD of the observation group patients were higher than those of the control group,while LVESD and LAD were lower than those of the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 13.79%,lower than 48.28%in the control group,and the difference was statistically significant(χ^(2)=8.056,P=0.005).Conclusion Mitral valvuloplasty is more effective in the treatment of rheumatic mitral valve diseas,and is worth popularizing in clinical treatment.
