摘　　要：目的探究hs-CRP、IL-6、Glu评估PFNA治疗股骨粗隆间骨折术后预后的应用价值。方法分析2019年3月至2022年3月海口市第四人民医院收治的102例股骨粗隆间骨折患者,根据PFNA术后的预后情况将患者分为预后良好组和预后不良组,收集患者的相关资料,分析影响患者的预后因素,对比两组患者的hs-CRP、IL-6及Glu水平,并绘制ROC曲线分析hs-CRP、IL-6、Glu评估患者预后的价值。结果单因素分析结果显示,年龄、术前ASA分级、骨折分型是影响PFNA治疗股骨粗隆间骨折术后预后的因素(P<0.05)。多因素分析结果显示,年龄、术前ASA分级、骨折分型是行PFNA治疗股骨粗隆间骨折患者预后的影响因素(P<0.05)。预后不良组患者的血清hs-CRP、IL-6及Glu水平明显高于预后良好组,差异具有统计学意义(t=4.160,3.349,4.939;P<0.05)。ROC曲线分析结果显示,hs-CRP、IL-6、Glu水平评估PFNA治疗股骨粗隆间骨折术后预后的曲线下面积分别为0.741、0.725、0.778,其中Glu单独预测价值最高,三者联合预测的曲线下面积为0.790。结论年龄、ASA分级、骨折分型均是PFNA治疗股骨粗隆间骨折术后预后的影响因素,hs-CRP、IL-6、Glu评估患者预后具有较高的价值。Objective To explore the application value of hs⁃CRP,IL⁃6 and Glu in the treatment of postoperative prognosis of PFNA in the treatment of intertrochanteric fractures.Methods A retrospective analysis was performed for 102 patients with intertrochanteric fracture admitted to the Fourth People's Hospital of Haikou from March 2019 to March 2022,and the patients were divided into the good prognosis group and the poor prognosis group according to the prognosis after PFNA surgery,relevant data of patients were collected,the prognostic factors affecting patients were analyzed,the levels of hs⁃CRP,IL⁃6 and Glu in the two groups were compared,and the ROC curves were drawn to analyze the value of hs⁃CRP,IL⁃6 and Glu to assess the prognosis of patients.Results The results of univariate analysis showed that age,preoperative ASA grade,and fracture classification were the factors affecting the prognosis of PFNA in the treatment of intertrochanteric fractures(P<0.05).The results of multivariate analysis showed that age,preoperative ASA grade,and fracture classification were the prognostic factors of patients with intertrochanteric fractures treated with PFNA(P<0.05).The serum levels of hs⁃CRP,IL⁃6 and Glu in the poor prognosis group were significantly higher than those in the good prognosis group(t=4.160,3.349,4.939;P<0.05).The results of ROC curve analysis showed that the prognostic areas under the curve for hs⁃CRP,IL⁃6 and Glu level evaluation of PFNA in the treatment of intertrochanteric fracture of the femur were 0.741,0.725 and 0.778,respectively,among which Glu alone had the highest predictive value,and the area under the curve predicted by the three was 0.790.Conclusion Age,ASA grade,and fracture classification were all influencing factors in the prognosis of PFNA in the treatment of intertrochanteric fractures,and hs⁃CRP,IL⁃6,and Glu have high value in evaluating the prognosis of patients.
