摘 要:目的探讨应用静息态fMRI和图论分析方法,分析冠心病患者脑功能网络的拓扑属性及其与认知功能的关系。方法选取70例冠心病患者(冠心病组)和50例年龄、性别和教育年限匹配的健康受试者(对照组),先采用简易智力状态量表(MMSE)、蒙特利尔认知量表(MoCA)评估其认知功能,再进行静息态fMRI扫描。图像预处理后利用Pearson correlation构建功能连接网络,并用图论方法分析其拓扑属性。采用统计学方法比较两组之间的认知评分、网络拓扑属性差异及其与认知功能的相关性。结果冠心病组MMSE、MoCA评分均较对照组明显降低,差异有统计学意义(P<0.001)。两组均表现出小世界网络,但是冠心病组聚类系数(Cp)较对照组更低,差异有统计学意义(P<0.05),而在全局效率(Eg)、局部效率(Eloc)、特征路径长度(Lp)、标准化聚类系数(γ)、标准化特征路径长度(λ)及小世界属性(σ)方面差异均无统计学意义(P>0.05)。冠心病组节点度在左侧眶部额上回较对照组增大,差异有统计学意义(P<0.05),在右侧颞上回和双侧颞中回较对照组减小,差异有统计学意义(P<0.05);冠心病组节点效率在左侧眶部额中回、右侧后扣带回及左侧海马较对照组明显增大,差异有统计学意义(P<0.05),而在右侧颞上回和左侧颞中回明显减小,差异有统计学意义(P<0.05)。冠心病组右侧后扣带回节点效率与MMSE评分呈正相关(P=0.038,r=0.214)。结论冠心病患者的脑功能网络拓扑属性已发生改变,尤其是右侧后扣带回节点效率改变与患者认知功能损害密切相关,提示fMRI可为探讨冠心病患者脑认知功能受损情况提供潜在的神经影像依据。Objective To investigate the topological properties of the functional connectome of patients with coronary artery disease(CAD)by using resting-state functional magnetic resonance imaging(rs-fMRI)and graph theory approaches and to evaluate their relations with cognitive function.Methods 70 patients with significant CAD and 50 healthy subjects who were matched for age,sex and years of education were recruited and evaluated cognitive function by Mini-Mental State Examination(MMSE)and Montreal Cognitive Assessment(MoCA),then,resting-state functional MR examination were performed.The whole-brain functional connectome was constructed by thresholding the Pearson correlation after data preprocessing and the topologic properties were analyzed by using graph theory approaches.Statistical analysis was used to assess the differences of cognitive scores and topological properties of the functional networks in both groups,and to investigate the correlation between these topological parameters and cognitive scores in CAD group.Results CAD patients had lower score in MMSE and MoCA than healthy controls(P<0.001).Both groups showed small-world networks,however,CAD group exhibited significantly lower in clustering coefficient(Cp)(P=0.038)while no differences in global efficiency(Eg),local efficiency(Eloc),characteristic path length(Lp),normalized clustering coefficient(γ),normalized characteristic path length(λ)and small-worldness(σ).The nodal degree of CAD group increased in left superior frontal gyrus(orbital part)and decreased in right superior temporal gyrus and bilateral middle temporal gyrus(P<0.05).The nodal of efficiency CAD group increased in left middle frontal gyrus(orbital part),left hippocampus and right posterior cingulate gyrus,and decreased in right superior temporal gyrus and left middle temporal gyrus(P<0.05).The nodal efficiency of the right posterior cingulate gyrus(PCG.R)was positively correlated with MMSE(P=0.038,r=0.214).Conclusion The functional connectome of CAD patients has changed in network and node l
