ADA、PGLYRP2及PCT在耐多药肺结核诊断中的潜在价值

摘　　要：目的探讨腺苷脱氨酶(ADA)、肽聚糖识别蛋白2(PGLYRP2)及降钙素原(PCT)在耐多药肺结核诊断中的潜在价值,以期为临床治疗提供参考。方法选取2019年5月—2022年1月在河南省禹州市疾病预防控制中心门诊就诊的110例耐多药肺结核患者作为耐药组,同期选择在本院门诊就诊的首次发现痰检结核杆菌阳性且无耐药史的肺结核患者110例作为敏感组。分析ADA、PGLYRP2、PCT在耐多药肺结核诊断中的潜在价值。结果耐药组患者PGLYRP2、PCT水平高于敏感组,而ADA水平低于敏感组,差异均有统计学意义(P<0.05)。ROC显示:ADA、PGLYRP2、PCT诊断耐多药肺结核的AUC分别为0.773(95%CI:0.700~0.845)、0.789(95%CI:0.707~0.871)、0.804(95%CI:0.669~0.939)。ADA的Cut-off=16.92 U/L、PGLYRP2的Cut-off=544.62 pg/mL、PCT的Cut-off=0.58 ng/mL,在该阈值下,诊断的灵敏度和特异度最高。结论PGLYRP2、PCT在耐多药肺结核患者体内呈高表达状况,ADA呈低表达状况,ADA、PGLYRP2、PCT水平与耐多药肺结核的发生存在相关性,可用于诊断耐多药肺结核。Objective This study aimed to investigate the potential value of adenosine deaminase(ADA),peptidoglycan recognition protein 2(PGLYRP2),and procalcitonin(PCT)in the diagnosis of multidrug-resistant pulmonary tuberculosis(MDR-TB).Methods A total of 110 patients with MDR-TB who visited the outpatient department of Yuzhou Center for Disease Control and Prevention from May 2019 to January 2022 were selected as the drug resistance group;110 patients with pulmonary tuberculosis found to have positive Mycobacterium tuberculosis in sputum examination for the first time without history of drug resistance in the outpatient department were selected as the sensitive group during the same period.The potential value of ADA,PGLYRP2,and PCT were analyzed in the diagnosis of multidrug-resistant pulmonary tuberculosis.Results The levels of ADA and PCT in the resistant group were significantly higher than those in the sensitive group,while the levels of PGLYRP2 were significantly lower than those in the sensitive group(P<0.05).Spearman correlation analysis showed that ADA and PCT levels were negatively correlated with the sensitivity of MDR-TB and positively correlated with PGLYRP2 levels in MDR-TB patients,with statistical significance(P<0.05).ROC showed that the area under the curve(AUC)of ADA,PGLYRP2,and PCT in the diagnosis of MDR-TB was 0.773(95%CI:0.700~0.845),0.789(95%CI:0.707~0.871)and 0.804(95%CI:0.669~0.939),respectively.The cut-off value was 16.92 U/L for ADA,544.62 pg/mL for PGLYRP2,and 0.58 ng/mL for PCT,with the highest sensitivity and specificity for diagnosis at this threshold.Conclusion PGLYRP2,and PCT are highly expressed in patients with MDR-TB.ADA,PGLYRP2,and PCT levels are correlated with the occurrence of MDR-TB,which can be used for detecting the occurrence of MDR-TB.
