血府逐瘀汤加减联合盐酸吗啡缓释片治疗肝癌疼痛瘀血内阻证疗效研究

摘　　要：目的:探讨血府逐瘀汤加减联合盐酸吗啡缓释片治疗肝癌疼痛瘀血内阻证临床疗效。方法:选取肝癌疼痛瘀血内阻证患者108例,按照随机信封法分为两组,对照组(n=54)给予盐酸吗啡缓释片治疗,观察组(n=54)给予血府逐瘀汤加减联合盐酸吗啡缓释片治疗。比较两组患者镇痛效果、疼痛评分、中医症状积分、镇痛因子以及生活质量。结果:治疗4周后观察组疼痛有效缓解率高于对照组(P<0.05)。治疗前、治疗2周后,两组患者数字疼痛分级量表(NRS)评分比较,差异无统计学意义(均P>0.05)。治疗3、4周后,观察组NRS评分低于对照组,差异有统计学意义(P<0.05)。治疗前两组患者肝区疼痛、腹内癥块、纳呆、情志抑郁中医症状积分比较,差异无统计学意义(均P>0.05)。治疗4周后观察组各项中医症状积分均低于对照组,差异有统计学意义(P<0.05)。治疗4周后观察组PGE2、NO低于对照组,差异有统计学意义(均P<0.05)。治疗4周后观察组QLQ-30量表各维度高于对照组,差异有统计学意义(均P<0.05)。结论:血府逐瘀汤加减联合盐酸吗啡缓释片治疗肝癌疼痛瘀血内阻证患者可降低镇痛因子水平,提升镇痛效果,缓解中医症状,改善生活质量。Objective:To explore clinical efficacy of modified Xuefu Zhuyu decoction supplemented with morphine hydrochloride sustained-release tablets in the treatment of patients with liver cancer pain of Yuxue Neizu syndrome.Methods:108 patients with liver cancer pain of Yuxue Neizu syndrome selected and divided into two groups according to the random envelope method,control group(n=54)morphine hydrochloride sustained-release tablets treatment and observation group(n=54)modified Xuefu Zhuyu decoction supplemented with morphine hydrochloride sustained-release tablets treatment.Comparisons were performed on the two groups in terms of analgesic effect,pain score,TCM symptoms scores,analgesic factors and quality of life.Results:After 4 weeks of treatment,the effective relief rate of pain in the observation group was higher than that in the control group(P<0.05).Before treatment and 2 weeks after treatment,there was no significant difference in the score of NRS between the two groups(all P>0.05).The NRS scores at 3 weeks and 4 weeks after treatment in the observation group were lower than those in the control group,difference statistically significant(P<0.05).There were no significant differences in the scores of TCM symptoms such as hepatalgia,intra-abdominal mass,anorexia and emotional depression between the two groups before treatment(all P>0.05).After 4 weeks of treatment,the above TCM symptoms scores in the observation group were lower than those in the control group,difference statistically significant(all P<0.05).After 4 weeks of treatment,the levels of PGE2 and NO in the observation group were lower than those in the control group(P<0.05).The above scores of QLQ-30 scale after 4 weeks of treatment were higher in the observation group than those in the control group,difference statistically significant(all P<0.05).Conclusion:Modified Xuefu Zhuyu decoction supplemented with morphine hydrochloride sustained-release tablets can reduce levels of body’s analgesic factors,enhance clinical analgesic effect,relieve TCM sympto
