电针夹脊穴联合祛风骨痛凝胶膏治疗2型糖尿病足气虚血瘀证临床研究

摘　　要：目的:观察电针夹脊穴联合祛风骨痛凝胶膏治疗2型糖尿病足气虚血瘀证患者的临床效果。方法:选择2型糖尿病足气虚血瘀证患者80例,以随机数字表法分为对照组(n=40)、治疗组(n=40)。治疗组采用电针夹脊穴联合祛风骨痛凝胶膏治疗;对照组应用电针夹脊穴治疗。比较两组患者临床症状积分、血液流变学指标、微循环血流情况。结果:治疗组的总有效率92.50%高于对照组的总有效率87.50%,差异有统计学意义(P<0.05)。治疗4周后,两组患者皮肤温度、疼痛程度、麻木程度、皮肤色泽、间歇性跛行距离的积分均较治疗前降低,且治疗组低于对照组,差异有统计学意义(均P<0.05)。治疗组血液流变学指标低于对照组,两组足背温度、温控血流、阻断后反应性充血、经皮氧分压均高于治疗前,且治疗组高于对照组,差异有统计学意义(均P<0.05)。结论:电针夹脊穴联合祛风骨痛凝胶膏能有效改善2型糖尿病足气虚血瘀证患者的临床症状,降低血液黏度,改善微循环血流状态,临床应用安全、作用持久。Objective:To observe clinical effect of electroacupuncture at Jiaji point combined with Qufeng Gutong gel ointment on patients with type 2 diabetic foot disease with Qixu Xueyu syndrome.Methods:Eighty patients of type 2 diabetic foot disease with Qixu Xueyu syndrome were randomly divided into control group(n=40)and treatment group(n=40).The treatment group were treated with electroacupuncture at Jiaji point combined with Qufeng Gutong gel ointment.The control group were treated with electroacupuncture at Jiaji point.The clinical symptom score hemorheological indexes and microcirculation blood flow were compared before and after treatment between the two groups.Results:The total effective rate of the treatment group 92.50%was higher than that in the control group 87.50%,difference statistically significant(P<0.05).After 4 weeks of treatment,the clinical symptom scores of skin temperature,pain,numbness,skin color,intermittent claudication distance in the two groups were lower than those before treatment,and the treatment group lower than those in the control group,difference statistically significant(all P<0.05).The hemorheology indexes in the treatment group were lower than those in the control group,and the dorsum temperature,temperature-controlled blood flow,reactive congestion after occlusion,and percutaneous partial oxygen pressure in both groups were higher than before treatment,the treatment group higher than those in the control group,difference statistically significant(all P<0.05).Conclusion:Electroacupuncture at Jiaji point combined with Qufeng Gutong gel ointment can effectively improve clinical symptoms of patients with type 2 diabetic foot disease with Qixu Xueyu syndrome,reducing blood viscosity and improving microcirculation blood flow state,which is safe and lasting in clinical application.
