摘 要:目的研究血小板抗体筛选及血小板交叉配型应用于血液病患者血小板输注中的效果。方法选取2018年8月至2020年7月厦门大学附属第一医院收治的84例血液病患者作为研究对象,按随机数字表法分为对照组和观察组,各42例。所有患者均输注血小板,对照组采取常规输注方法,观察组于血小板输注前采集患者3 ml静脉血,以固相凝集法检测血小板抗体,针对检测结果呈阳性患者,以固相凝集法实施血小板交叉配型实验,针对检测结果呈阴性患者实施常规输注,比较两组血小板输注效果、输注后血小板计数增加指数(CCI)水平。结果观察组输注有效率为95.24%,高于对照组的76.19%,差异有统计学意义(P<0.05);观察组输注后1、24 h CCI水平均高于对照组,差异有统计学意义(P<0.05)。结论血小板抗体筛选及血小板交叉配型应用于血液病患者血小板输注,可有效增强输注效果,以保证临床治疗效果。Objective To study the effect of platelet antibody screening and platelet cross-matching in platelet transfusion in patients with hematological diseases.Methods A total of 84 patients with hematological diseases who were admitted to the First Affiliated Hospital of Xiamen University from August 2018 to July 2020 were selected as the research subjects,and they were divided into control group and observation group according to the random number table method,with 42 cases in each group.All patients were transfused with platelets,the control group was given routine transfusion,3 ml venous blood was collected from the patients in the observation group before platelet transfusion,and platelet antibodies were detected by solid-phase agglutination,routine transfusion was carried out,the effect of platelet transfusion and the levels of platelet count increase index(CCI)were compared between the two groups.Results The effective rate of infusion in the observation group was 95.24%,which was higher than 76.19%in the control group,and the difference was statistically significant(P<0.05).1,24 h after influsion,the levels of CCI in the observation group was higher than that in the control group,and the difference was statistically higher(P<0.05).Conclusion Platelet antibody screening and platelet cross-matching can be applied to platelet transfusion in patients with hematological diseases,which can effectively improve the transfusion effect and ensure the clinical treatment effect.
