摘　　要：目的分析维持性血液透析(MHD)患者中心静脉狭窄相关危险因素。方法回顾性分析2015年8月至2020年8月本院收治的中心静脉狭窄的92例MHD患者作为观察组,另选取同期本院收治的非中心静脉狭窄的93例MHD患者作为对照组。收集并分析两组的临床资料及实验室指标;多因素Logistic回归分析中心静脉狭窄的危险因素。结果单因素分析结果显示,两组载脂蛋白A(ApoA)、脂蛋白a[LP(a)]、甲状旁腺素、CRP水平比较差异有统计学意义(P<0.05)。多因素Logistic分析结果显示,ApoA、LP(a)、甲状旁腺素及CRP水平是中心静脉狭窄的危险因素,差异有统计学意义(P<0.05)。结论MHD患者中心静脉狭窄与患者的ApoA、LP(a)、甲状旁腺素、CRP水平密切相关,通过预防以上危险因素降低MHD患者中心静脉狭窄的发生风险。Objective To analyze the risk factors related to central vein stenosis in maintenance hemodialysis patients(MHD).Methods Retrospective analysis of 92 MHD patients with central vein stenosis admitted to our hospital from August 2015 to August 2020 as the observation group,Another 93 MHD patients without central venous stenosis admitted to our hospital during the same period were selected as the control group.The clinical data and laboratory indexes of the two groups were collected and analyzed;multivariate Logistic regression was used to analyze the risk factors of central venous stenosis.Results Univariate analysis showed that there were significant differences in the levels of Apolipoprotein-A(ApoA),lipoprotein a[LP(a)],parathyroid hormone,and CRP between the two groups(P<0.05).Multivariate Logistic analysis showed that the levels of ApoA,LP(a),parathyroid hormone and C-reactive protein(CRP)were risk factors for central vein stenosis,and the difference was statistically significant(P<0.05).Conclusion Central venous stenosis in MHD patients is closely related to the levels of ApoA,LP(a),PTH and CRP.The risk of central venous stenosis in MHD patients can be reduced by preventing the above risk factors.
