右美托咪定联合布托啡诺对机械通气患者镇静镇痛的效果

摘　　要：目的探究右美托咪定(Dex)联合布托啡诺(BT)对机械通气患者的镇静、镇痛效果。方法选取2020年5月至2021年5月于本院ICU行机械通气的110例患者作为研究对象,采用随机数字表法分为对照组和观察组,各55例。对照组采用Dex镇静镇痛,观察组采用Dex联合BT镇静镇痛,比较两组镇静镇痛效果[镇静躁动评分(RASS)、重症监护疼痛观察工具(CPOT)、镇痛时间、镇静时间、清醒时间]、不良反应发生情况。结果观察组RASS、CPOT评分均低于对照组,且镇痛时间、镇静时间、清醒时间均短于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率为1.81%,低于对照组的12.72%,差异有统计学意义(P<0.05)。结论Dex联合BT治疗机械通气患者的镇静镇痛效果显著,能减少不良反应的发生,值得临床推广应用。Objective To investigate the sedative and analgesic effects of dexmedetomidine(Dex)combined with butorphanol(BT)in patients with mechanically ventilation.Methods A total of 110 patients who underwent mechanical ventilation in the ICU of our hospital from May 2020 to May 2021 were selected as the study subjects,and they were divided into control group and observation group according to random number table method,with 55 cases in each group.The control group was sedative and analgesic with Dex,and the observation group was sedative and analgesic with Dex combined with BT,the effects of sedation and analgesic[richmond agitation sedation scale(RASS),critical-care pain observation tool(CPOT),analgesic time,sedation time,awake time]and the occurrence of adverse reactions were compared between the two groups.Results The scores of RASS and CPOT in the observation group were lower than those in the control group,and the analgesia time,sedation time and awake time were shorter than those in the control group,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 1.81%,which was lower than 12.72%in the control group,and the difference was statistically significant(P<0.05).Conclusion Dex combined with BT has significant effect on sedation and analgesia in mechanically ventilated patients,which can reduce the occurrence of adverse reactions,and is worthy of clinical promotion and application.
