协同护理模式对慢性心力衰竭患者容量管理及自护行为的影响

摘　　要：目的探讨协同护理模式(CCM)对慢性心力衰竭(CHF)患者容量管理及自护行为的影响。方法选取2019年7—9月本院收治的82例CHF患者作为研究对象,采用随机抽样法分为对照组和观察组,每组41例。对照组采用常规护理措施,观察组在对照组基础上采用CCM,比较两组临床客观指标及入院前后主观评价指标。结果两组电解质紊乱发生率比较差异无统计学意义;观察组住院期间日平均体质量控制低于对照组、住院时间短于对照组,3个月内再入院率少于对照组,差异有统计学意义(P<0.05)。入院时,两组欧洲心力衰竭患者自我护理行为量表(EHFScB-9)评分比较差异无统计学意义;出院时,两组EHFScB-9评分均低于入院时,且观察组低于对照组,差异有统计学意义(P<0.05)。结论CCM强调了护士-患者-患者家属三方之间协同护理的关系,激发了慢性心力衰竭患者及其家属的积极能动性,有利于其容量管理,并有助于提高患者自护行为能力,缩短其住院时间并降低3个月内再入院率,临床护理效果明显。Objective To investigate the effects of collaborative care mode(CCM)on volume management and self-care behavior in patients with chronic heart failure(CHF).Methods A total of 82 patients with CHF admitted to our hospital from July to September 2019 were selected as the research subjects,and they were divided into control group and observation group by random convenience sampling,with 41 cases in each group.The control group was given routine nursing measures,and the observation group was given CCM on the basis of the control group,and the clinical objective indicators and subjective evaluation indicators before and after admission were compared between the two groups.Results There was no significant difference in the incidence of electrolyte disorde between the two groups,the average daily weight control during hospitalization in the observation group was lower than that of the control group,the number of hospital stays was shorter than that of the control group,and the readmission rate within 3 months was less than that of the control group,and the difference was statistically significant(P<0.05).On admission,there was no statistically significant difference in the scores of the European heart failure self-care behavior scale(EHFScB-9)between the two groups;at discharge,the EHFScB-9 scores in both groups were lower than those at admission,and the observation group was lower than that in the control group,and the difference was statistically significan(P<0.05).Conclusion The CCM emphasizes the collaborative nursing relationship between nurse-patient-patient-family members,stimulates the positive initiative of patients with chronic heart failure and their families,is conducive to their volume management,helps to improve the self-care behavior ability of patients with chronic heart failure,shortens their hospital stay and reduces the readmission rate within 3 months,and the clinical nursing effect is obvious.
