带蒂皮瓣术后14天断蒂修复手外伤皮肤缺损的效果评估

摘　　要：目的评估带蒂皮瓣术后14 d断蒂修复手外伤皮肤缺损的效果。方法选取2020年1月至2021年2月本院收治的70例手外伤皮肤缺损患者作为研究对象,根据断蒂时间不同分为对照组和研究组,各35例。对照组于21 d进行断蒂,研究组于14 d进行断蒂,比较两组修复手外伤皮肤缺损效果及断蒂后恢复情况,治疗费用情况和住院时间,手术前后生命质量评分。结果研究组修复手外伤皮肤缺损优良率高于对照组,但两组比较差异无统计学意义;两组断流试验正常、针刺皮瓣出血、毛细血管反应正常、皮瓣颜色正常、皮瓣存活情况比较差异无统计学意义。术前,两组生命质量评分比较差异无统计学意义;术后,两组生命质量评分均高于治疗前,且研究组高于对照组(P<0.05);研究组住院费用低于对照组,住院时间短于对照组,差异有统计学意义(P<0.05)。结论在手外伤皮肤缺损患者实施修复中,带蒂皮瓣术后14 d断蒂修复效果较理想,值得临床推广应用。Objective To evaluate the effect of 14 days after pedicle flap surgery to repair hand traumatic skin defects.Methods 70 patients with hand trauma skin defects from January 2020 to February 2021 admitted to our hospital were selected as the study subjects,and they were divided into control group and research group according to the time of pedicle cut,with 35 cases in each group.The control group underwent pedicle amputation at 21 d,and the research group underwent pedicle amputation at 14 d,and the effect of repairing skin defects from hand trauma and the recovery after pedicure,treatment cost,length of hospital stay,and quality of life before and after surgery were compared between the two groups.Results The excellent rate of repairing skin defects of hand trauma in the research group was higher than that in the control group,and there was no significant difference between the two groups.There was no significant difference between the two groups in normal flow cut-off test,needle flap bleeding,normal capillary reaction,normal flap color,and flap survival.Before surgery,there was no significant difference in quality of life scores between the two groups;after surgery,the quality of life scores in the two groups were higher than before treatment,and the research group was higher than that of the control group(P<0.05).The hospitalization costs of the research group was lower than that of the control group,and the length of hospital stay was shorter than that of the control group,and the difference was statistically significant(P<0.05).Conclusion In the repair of skin defects with hand traumatic,the repair effect of broken peduncle 14 days after pedicle flap surgery is ideal,which is worthy of clinical promotion and application.
