儿童感染性疾病诊断中血清白细胞介素-6与血清降钙素原及联合诊断的应用价值

摘　　要：目的分析儿童感染性疾病血清白细胞介素-6(IL-6)与血清降钙素原(PCT)及联合诊断的应用价值。方法选取2018年1月至2020年12月本院收治的142例感染性疾病儿童作为观察组,另选取同期本院收治的150例非感染性疾病儿童作为对照组,两组均实施血液细菌培养、血清IL-6、血清PCT检查。比较两组IL-6、PCT水平;分析血清PCT、IL-6及联合诊断的结果,比较血清PCT、IL-6及联合诊断的应用效果。结果观察组IL-6、PCT水平均高于对照组,差异有统计学意义(P<0.05)。142例儿童感染患者中,IL-6诊断结果为阳性140例,阴性152例;PCT诊断结果为阳性139例,阴性153例;联合诊断结果为阳性142例,阴性150例。血清PCT、IL-6及联合诊断灵敏度、准确度、特异度、阳性预测值及阴性预测值比较差异有统计学意义(P<0.05);联合诊断灵敏度、准确度、阳性预测值及阴性预测值均高于血清PCT、IL-6,特异度高于IL-6,差异有统计学意义(P<0.05)。结论对于感染性疾病患者,可以采取血清PCT指标诊断联合血清IL-6指标诊断方式判断疾病有无及感染类型,为儿童患者开展后续感染性疾病干预治疗提供相关指导方向。Objective To analyze the appliation value of serum interleukin-6(IL-6)and serum procalcitonin(PCT)and combined diagnosis of infectious diseases in children.Methods 142 children with infectious diseases admitted to our hospital from January 2018 to December 2020 were selected as the observation group,and 150 children with non-infectious diseases admitted to our hospital during the same period were selected as the control group.Blood bacterial culture,serum IL-6 and serum PCT were performed in both groups.The levels of IL-6 and PCT were compared between the two groups,the results of serum PCT,IL-6 and combined diagnosis were analyzed,and the application effects of serum PCT,IL-6 and combined diagnosis were compared.Results The levels of IL-6 and PCT in observation group were significantly higher than those in control group,and the difference was statistically significant(P<0.05).Among 142 children with infection,140 cases were IL-6 positive and 152 cases were IL-6 negative.PCT diagnostic results were positive in 139 cases and negative in 153 cases.The combined diagnosis results were 142 positive cases and 150 negative cases.There were significant differences in the sensitivity,accuracy,specificity,positive predictive value and negative predictive value of serum PCT,IL-6 and combined diagnosis(P<0.05);the sensitivity,accuracy,positive predictive value and negative predictive value of combined diagnosis were higher than those of serum PCT and IL-6,and the specificity was higher than that of IL-6,and the differences were statistically significant(P<0.05).Conclusion For patients with infectious diseases,serum PCT index diagnosis combined with serum IL-6 index can be used to determine the disease presence and the type of infection,so as to provide relevant guidance for child patients to carry out subsequent intervention and treatment of infectious diseases.
