摘　　要：目的探究中心静脉导管胸腔闭式引流在结核性胸膜炎治疗中的应用效果。方法选取2020年1—12月本院收治的64例结核性胸膜炎患者作为研究对象,按随机数字表法分为A组和B组,每组32例。A组实施胸腔穿刺术治疗,B组实施中心静脉导管胸腔闭式引流治疗。比较两组临床疗效、临床指标(总抽液量、胸膜厚度、胸腔积液吸收时间、住院时间)及并发症发生情况。结果B组治疗总有效率为96.87%,明显高于A组的78.12%,差异有统计学意义(P<0.05)。B组总抽液量明显多于A组,胸膜厚度明显薄于A组,胸腔积液吸收时间、住院时间均明显短于A组,差异有统计学意义(P<0.05)。B组发生并发症发生率为3.13%,明显低于A组的18.76%,差异有统计学意义(P<0.05)。结论中心静脉导管胸腔闭式引流治疗结核性胸膜炎疗效显著,可缩短胸腔积液吸收时间,减轻患者胸穿的痛苦,减少并发症的发生,值得临床推广应用。Objective To explore the effect of closed thoracic drainage of central venous catheter in the treatment of tuberculous pleurisy.Methods 64 patients with tuberculous pleurisy admitted to our hospital from January to December 2020 were selected as the research subjects,and they divided into group A and group B according to the random number table method,with 32 cases in each group.Group A was treated with thoracentesis,and group B was treated with closed thoracic drainage with central venous catheter.The clinical efficacy,clinical indicators(total fluid extraction volume,pleural thickness,pleural effusion absorption time,hospital stay)and complications were compared between the two groups.Results The total effective rate in group B was 96.87%,which was significantly higher than 78.12%in group A,the difference was statistically significant(P<0.05).The total fluid extraction volume in group B was significantly more than that in group A,the pleural thickness was significantly thinner than that in group A,the absorption time of pleural effusion and the length of hospital stay were significantly shorter than those of group A,and the difference was statistically significant(P<0.05).The incidence of complications in group B was 3.13%,significantly lower than 18.76% in group A,and the difference was statistically significant(P<0.05).Conclusion Central venous catheter thoracic closed drainage is effective in the treatment of tuberculous pleurisy,which can shorten the absorption time of pleural effusion,reduce the pain of patients with chest puncture,and reduce the occurrence of complications,which is worthy of clinical application.
