摘　　要：信息化技术在健康领域的发展,给慢性病管理带来了契机。本文通过介绍患者门户(PPS)这项信息化工具的国内外应用现状、发展趋势和作用、基于该系统的智慧慢病管理的内容和要求,分析我国慢病管理的现状和存在的问题,提出利于我国慢病管理的远程医疗服务模式和移动干预方案,以期为慢病信息化发展和防控提供参考。The development of information technology in the field of health brings an opportunity for chronic-disease management.This paper introduces the application status,development trend and function of patient portal system(PPS),an information tool at home and abroad,as well as the content and requirements of intelligent chronic disease management based on this system,analyzes the current situation and existing problems of chronic disease management in China,and puts forward the telemedicine service mode and mobile intervention scheme conducive to chronic disease management in China,in order to provide reference for the development and prevention and control of chronic disease information.
