7例导管型及囊肿型浆细胞性乳腺炎稳定性的回顾性分析

摘　　要：目的探讨导管型及囊肿型浆细胞性乳腺炎的声像图特征及其转归,为临床诊治提供参考。方法回顾性分析2018年3月至2022年2月于本院长期复查的7例导管型及囊肿型浆细胞性乳腺炎患者的临床资料,分析超声特征、病灶稳定期和急性发作的早期超声特征。结果浆细胞性乳腺炎导管型及囊肿型稳定期的特点为肿块壁不规则增厚,呈均匀、较高回声,内外壁清晰,以内壁清晰、连续、呈线状多见。稳定病灶局部血流丰富,回声减低,透声变差,内壁不光滑或呈带状增厚,提示病灶可能急性发作。当病灶回声减低并外渗时治疗效果不佳。结论导管型及囊肿型浆细胞性乳腺炎超声图像有其独特的特征,超声可作为稳定期病灶的监测手段,能及时发现稳定期病灶急性发作早期的超声特征,有利于临床及时采取干预措施。Objective To investigate the sonogram characteristics and prognosis of ductal and cystic plasma cell mastitis,and to provide reference for clinical diagnosis and treatment.Methods The clinical data of 7 patients with ductal and cystic plasmacytic mastitis who were reviewed in our hospital for a long time from March 2018 to February 2022 were retrospective analyzed and the ultrasound features,stable lesions and early ultrasound features of acute onset were analyzed.Results The ductal and cystic stabilization stages of plasma cell mastitis were characterized by irregular thickening of the mass wall,uniform and high echo,clear inner and outer walls,and clear,continuous and linear inner walls.Stable lesions with abundant local blood flow,decreased echo,poor sound transmission,and unsmooth or band-like thickening of the inner wall,suggesting that the lesion may have an acute onset.Treatment is not effective when lesion echo is reduced and extravasation.Conclusion The ultrasound images of ductal and cystic plasma cell mastitis have their own unique characteristics,and ultrasound can be used as a monitoring method for stable lesions,which can timely detect the ultrasound features of stable lesions in the early stage of acute onset,which is conducive to timely clinical interventions.
