同期腔内手术治疗输尿管中下段结石合并前列腺增生的有效性及安全性

摘　　要：目的探究同期腔内手术治疗输尿管中下段结石合并前列腺增生的有效性及安全性。方法选取2018年5月至2020年5月于本院就诊的102例输尿管中下段结石合并前列腺增生患者作为研究对象,依照奇偶次序分为对照组和实验组,每组51例。对照组行分期腔内手术,实验组行同期腔内手术,比较两组手术情况(碎石成功率、手术时间、住院时间)、并发症发生情况、国际前列腺症状评分(IPSS)、生活质量评分(QOL)及尿动力学指标[最大尿流率(Qmax)、残余尿量(PVR)]。结果两组碎石成功率、手术时间、并发症总发生率比较差异无统计学意义;实验组住院时间短于对照组,差异有统计学意义(P<0.05)。手术前后,两组IPSS评分、QOL评分、Qmax、PVR比较差异无统计学意义;术后,两组IPSS评分均低于术前,QOL评分、Qmax均高于术前,PVR均少于术前,差异有统计学意义(P<0.05)。结论采用同期腔内手术治疗输尿管中下段结石合并前列腺增生安全有效,且在手术操作前需全面评估患者病情,便于手术顺利进行,值得临床推广应用。Objective To investigate the efficacy and safety of simultaneous endoluminal surgery in the treatment of middle and lower ureteral stones and prostatic hyperplasia.Methods A total of 102 patients with middle and lower ureteral stones and prostatic hyperplasia who visited our hospital from May 2018 to May 2020 were selected as the research subjects,and they were divided into control group and experimental group in order of parity,with 51 cases in each group.The surgical conditions(lithotripsy success rate,operation time,hospitalization time),complication occurrence,international prostate symptom score(IPSS),quality of life score(QOL),urodynamic indexes(maximum urine flow rate[Qmax],residual urine volume[PVR])were compared between the two groups.Results There were no significant differences in the success rate,operation time and total complication rate between the two groups.The hospitalization time of the experimental group was shorter than that of the control group,and the difference was statistically significant(P<0.05).Before and after surgery,there were no significant differences in IPSS score,QOL score,Qmax and PVR between the two groups;after surgery,the IPSS scores of the two groups were lower than that before surgery,the QOL score and the Qmax of the two groups was higher than those before surgery,and the PVR was lower than that before surgery,the difference was statistically significant(P<0.05).Conclusion The use of simultaneous endoluminal surgery for the treatment of middle and lower ureteral stones combined with prostatic hyperplasia is safe and effective,and the patient's condition needs to be fully evaluated before the operation,which is convenient for the smooth operation and is worthy of clinical promotion and application.
