25-羟维生素D_(3)和胰岛素样生长因子-1及脯氨酸肽酶水平与糖尿病性周围神经病变的相关性分析

摘　　要：目的分析25-羟维生素D_(3)[25-(OH)D_(3)]、胰岛素样生长因子-1(IGF-1)、脯氨酸肽酶(PLD)水平与糖尿病性周围神经病变的相关性。方法选取2018年1月至2019年1月本院收治的83例糖尿病性周围神经病变患者纳入观察组,另选取同期83例单纯糖尿病患者纳入对照组。两组均检测PLD、25-(OH)D_(3)、IGF-1、血糖指标[空腹血糖(FBG)、糖化血红蛋白(HbA1c)]水平,比较两组PLD、25-(OH)D_(3)、IGF-1和血糖水平,比较观察组不同严重程度糖尿病性周围神经病变患者PLD、25-(OH)D_(3)、IGF-1和血糖水平;分析PLD、25-(OH)D_(3)、IGF-1水平与糖尿病性周围神经病变程度的相关性。结果观察组PLD、25-(OH)D_(3)、IGF-1水平均低于对照组(P<0.05);观察组不同病情程度患者PLD、25-(OH)D_(3)、IGF-1水平比较差异有统计学意义(P<0.05),重度患者25-(OH)D_(3)、IGF-1水平均低于轻度、中度患者,且中度患者低于轻度患者(P<0.05);轻度、中度和重度患者PLD、25-(OH)D_(3)、IGF-1水平均低于对照组(P<0.05)。两组FBG水平比较差异无统计学意义;观察组不同病情程度患者FBG水平比较差异无统计学意义;观察组HbA1c水平高于对照组(P<0.05);观察组不同病情程度患者HbA1c水平比较差异有统计学意义(P<0.05),重度患者HbA1c水平均高于中度、轻度患者,且中度患者高于轻度患者(P<0.05);中度、重度患者HbA1c水平均高于对照组(P<0.05)。Spearman相关性分析显示,PLD、25-(OH)D_(3)、IGF-1水平与糖尿病性周围神经病变程度呈负相关(r=-0.683、-0.691、-0.746,P<0.05)。结论糖尿病性周围神经病变患者PLD、25-(OH)D_(3)、IGF-1水平低于糖尿病患者,且其与糖尿病性周围神经病变严重程度呈负相关,各指标水平降低,糖尿病性周围神经病变程度越严重。Objective To analyze the correlation of 25-hydroxyvitamin D_(3)(25-[OH]D_(3)),insulin like growth factor-1(IGF-1)and prolidase(PLD)levels with diabetic peripheral neuropathy.Methods 83 patients with diabetic peripheral neuropathy in our hospital from January 2018 to January 2019 were enrolled as observation group,meantime,another 83 patients with simple diabetes were set as control group.The levels of PLD,25-(OH)D_(3),IGF-1,blood glucose index(fasting blood glucose[FBG],and glycosylated hemoglobin A1c[HbA1c])were detected in both groups.The levels of PLD,25-(OH)D_(3),IGF-1 and blood glucose index were compared between two groups,and the comparison was also conducted among patients with diabetic peripheral neuropathy of different severity degrees.Thereafter,the correlation of PLD,25-(OH)D_(3) and IGF-1 with the severity of diabetic peripheral neuropathy was analyzed.Results The levels of PLD,25-(OH)D_(3) and IGF-1 in the observation group were lower than those in the control group(P<0.05);there were significant differences in the levels of PLD,25-(OH)D_(3) and IGF-1 in patients with different disease degrees in the observation group(P<0.05),the levels of 25-(OH)D_(3) and IGF-1 in severe patients were lower than those in moderate and mild patients,and the moderate patients were lower than those in mild patients(P<0.05);the levels of PLD,25-(OH)D_(3) and IGF-1 in mild,moderate and severe patients were lower than those in the control group(P<0.05).There was no statistical difference in FBG levels between the two groups,and there was no significant difference in FBG levels in patients with different disease degrees in the observation group;the level of HbA1c in the observation group was higher than that in the control group(P<0.05).There were significant differences in HbA1c levels in patients with different severity degrees in the observation group(P<0.05),the HbA1c levels in the severe patients was higher than that of moderate and mild patients,and the moderate patients was higher than that of mild patients(P<0.05);
